摘  要:目的:对比分析1.5T MRI与关节镜检查对前交叉韧带(anterior cruciate ligament,ACL)损伤诊断的一致性。方法:对162例ACL损伤患者行MRI检查,以关节镜结果作为诊断标准。采用受试者工作特征(receiver operating characteristic,ROC)曲线评价MRI对ACL的诊断效能;对比分析两者诊断结果的一致性。结果:MRI诊断膝关节ACL损伤灵敏度为98.69%,特异度为81.82%,准确率为98.77%;两者的诊断结果具有高度一致性(Kappa值=0.909,P>0.05)。结论:MRI诊断ACL损伤具有很高的灵敏度和较好的特异性,与关节镜具有较好的诊断一致性,可以作为诊断膝关节ACL损伤首选的非创伤性的检查方法。Objective To analyze the consistency between 1.ST MRI and arthroscope when used to diagnose anterior cruciate ligament (ACL) injuries. Methods Totally 162 ACL injury patients underwent MRI examination, and the results by arthroscope were used as the diagnosis standard. The curve of receiver operating characteristic (ROC) was applied to evaluating the ACL injuries diagnosis by MRI, and the consistency was analyzed between 1.5T MRI and arthroscope. Results MRI had the sensitivity, specificity and accuracy being 98.69%, 81.82% and 98.77% respectively when used to diagnose ACL injuries, and there was high consistency between MRI and arthroscope (Kappa=0.909, P〉0.05). Conclusion MR1 gains high sensitivity and specificity when used to diagnose ACL injuries as well as high consistency with arthroscope, and thus can be a preferred non-invasive way to diagnose ACL injuries.
