套扎器改良内扎内注与吻合器痔切闭治疗Ⅲ、Ⅳ期痔的疗效比较

摘　　要：目的:比较套扎器改良内扎内注术与吻合器痔切闭术治疗Ⅲ、Ⅳ期混合痔的临床效果。方法:选取本院肛肠科符合Ⅲ、Ⅳ期混合痔诊断并住院治疗的患者140例,按照随机数字表法分为治疗组和对照组,各70例。治疗组采用套扎器改良内扎、内注术,对照组采用吻合器行痔切闭术。观察比较两组的治疗效果与术后术口疼痛、出血、肛门坠胀、肛门狭窄、肛乳头增生等并发症指标。结果:治疗组痊愈率为97.14%(68/70),对照组为91.43%(64/70),两组比较差异无统计学意义(P>0.05)。治疗组术后术口疼痛、出血、肛门坠胀、肛门狭窄、肛乳头增生的并发症发生率均明显低于对照组,差异均有统计学意义(P<0.05)。结论:套扎器改良内扎内注术治疗Ⅲ、Ⅳ期混合痔,疗效满意,术后并发症少,值得临床推广。Objective： To compare the clinical efficacy of ligation device modified internal ligation injection and stapler hemorrhoidectomy on the treatment of stage III and IV mixed hemorrhoids.Method ： A total of 140 patients with stage Ⅲ and Ⅳ mixed hemorrhoids in our hospital were selected and divided into the treatment group and the control group according to random number table method, 70 cases in each group.the treatment group was treated with ligation device modified internal ligation injection, The control group was treated with stapler hemorrhoidectomy.The clinical effect and complications such as pain, bleeding, anal bulge, anal stenosis and anal papillary hyperplasia of two groups were observed and compared.Result： The cure rate of the treatment group and the control group were 97.14% （ 68/70 ） and 91.43% （ 64/70 ） respectively, the difference had no statistical significance （P〉0.05） .The complications such as pain, hemorrhage, anal bulge, anal stenosis and anal papillary hyperplasia of the treatment group were significantly lower than those of the control group, the differences were statistically significance （P〈0.05） .Conclusion： The ligation device modified internal ligation injection for treatment of stage Ⅲ and Ⅳ mixed hemorrhoids has satisfactory curative effect and few postoperative complications, it is worthy to be popularized in clinical practice.
