摘 要:目的:探讨一氧化氮(NO)、内皮素(ET)在颅脑损伤患者脑脊液中的表达及意义。方法:选取120例在本院住院时间超过7 d(含7 d)的颅脑损伤患者,根据GCS评分分为轻度组、中度组与重度组。采用Griess比色法和均相竞争法分别测定患者脑脊液中一氧化氮与内皮素水平。结果:120例颅脑损伤患者脑脊液中的内皮素水平高于正常参考值(P<0.05),一氧化氮水平低于正常参考值(P<0.05)。中度组患者入院第1天的一氧化氮、内皮素水平均高于轻度组,差异均有统计学意义(P<0.05);重度组的一氧化氮、内皮素水平均高于中度组,差异均有统计学意义(P<0.05)。颅脑损伤患者入院第1天脑脊液中内皮素水平达到最高峰值(195.96±4.38)pg/m L,一氧化氮水平同样达到峰值(13.52±6.01)μmol/L,在入院第3天,这两项指标均逐渐下降,与入院时比较,差异均有统计学意义(P<0.05)。结论:脑脊液中一氧化氮、内皮素水平的测定能较好地评估患者的颅脑损伤程度。Objective: To study the expression and significance of nitric oxide ( NO ) and endothelin ( ET ) in cerebrospinal fluid ( CSF ) of patients with brain injury.Method: A total of 120 craniocerebral injury patients with hospitalization beyond 7 days ( including 7 days ) in our hospital were divided into the mild group, the moderate group and the severe group according to GCS scores.Endothelin and nitric oxide levels of 120 cases were respectively detected by the Griess colorimetric method and the homogeneous competition method.Result: The endothelin level in eerebrospinal fluid of 120 patients with craniocerebral injury was higher than that normal reference value ( P〈0.05 ), the level of nitric oxide was lower than that normal reference value ( P〈0.05 ) .The levels of nitric oxide and endothelin in first day in the moderate group were higher than those in mild group, the differences were statistically significant ( P〈0.05 ) ; the levels of nitric oxide and endothelin in the severe group were higher than those in the moderate group, the differences were statistically significant (P〈0.05) .On the first day after entering the hospital eerebrospinal fluid endothelin level reached the peak ( 195.96 ± 4.38 ) pg/mL, and nitric oxide level also reached the peak ( 13.52 ± 16.01 ) p.mol/L, and after 3 days in the hospital, both of them gradually decreased, compared with admission, the differences were statistically significant (P〈0.05) . Conclusion: The level of nitric oxide and endothelin in cerebrospinal fluid can be used to evaluate the degree of brain injury.
