摘  要:目的观测人参总皂苷(TG)对急性心肌梗死(AMI)大鼠心功能和血流动力学及心室重构的影响。方法雄性SD大鼠,结扎冠状动脉前降支建立AMI模型,假手术组、模型组、TG高剂量(TG-H)和低剂量组(TG-L)大鼠分别腹腔注射等量生理盐水、TG 40 mg/(kg·d)和20 mg/(kg·d)35 d。彩色多普勒超声心动图动态监测心功能变化,第35天经颈动脉插管至左心室检测心脏血流动力学指标,肉眼观、HE和Masson染色观察心肌重构变化。结果与模型组比较,第7、14、35天TG-H组LVEF和LVFS均显著升高(P<0.05,P<0.01),而TGL组仅于第35天显著升高(P<0.05);第35天TG-H组LVEDd、LVESd、LVEDV和LVESV均明显低于模型组(P<0.05);且TG-H和TG-L组LVSP、±dp/dt_(max)均显著高于模型组(P<0.05,P<0.01),LVEDP明显低于模型组(P<0.05);TG-H、TG-L组心室扩张、梗死区面积和纤维化瘢痕较模型组减少,左室壁较厚,心肌细胞较多。结论人参总皂苷明显减轻梗死区瘢痕形成及心室重构,从而显著改善AMI后大鼠心功能和血流动力学指标。Objective To investigate the effects of total ginsenosides (TG) on cardiac function and ventricu- lar remodeling of rats with acute myocardial infarction ( AMI). Methods AMI models were made by the left coronary artery ligated in male Sprague Dawley rats. The rats of sham, model, TG high dose (TG-H) and low dose (TG-L) groups were injected into abdominal cavity with equal-volume of normal saline,TG 20 mg/(kg-d) and 40 mg/(kg-d) for 35 days, respectively. The change of heart functions were dynamically monitored by color doppler ultrasound. Heart bemodynamic parameters of rats in each group were detected via carotid artery intubation to the left ventricle at the 35th day after ligation. Gross appearance, HE and Masson staining were used to observe the pathological changes of myocar- dial remodeling. Results At the 7th,14th and 35th day after ligation,compared with model group,the left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) were increased in TG-H group ( P 〈 0. 05, P 〈0. 01 ) ,and LVEF and LVFS were obviously increased in TG-L group at the 35th day after ligation (P 〈0. 05). At the 35th day, left ventricular end-diastolic dimension, left ventricular end-systolic dimension, left ventricular end-dias- tolic volume and left ventricular end-systolic volume were all significantly reduced in TG-H group compared with mod- el group ( P 〈 0. 05 ). At the 35th day after ligation, the left ventricular systolic pressure and left ventricular pressure maximum rise and fall rate ( ~ dp/dtmax ) were all obviously increased (P 〈 0. 05, P 〈 0. 0l ) in TG groups compared with model group. But left ventricular end-diastolic pressure was reduced in TG groups compared with model group ( P 〈0. 05). Compared with model group,the ventricular dilatation,infarct area and fibrosis scars markedly decreased in TG-H and TG-L groups. Left ventricular wall was thicker, and myocardial cells were more.
