半夏白术天麻汤联合西药治疗痰浊中阻椎-基底动脉供血不足性眩晕随机平行对照研究

摘　　要：[目的]观察半夏白术天麻汤联合西药治疗痰浊中阻椎-基底动脉供血不足性眩晕疗效。[方法]使用随机平行对照方法,将69例住院患者按就诊号顺序随机分为两组。对照组35例长春西汀30mg+0.9%氯化钠250m L,静滴,1次/d;尼莫地平,40mg/次,3次/d,口服。治疗组34例半夏白术天麻汤(姜半夏10g,炒白术15g,天麻12g,茯苓20g,陈皮15g,当归10g,川芎15g,党参18g,柴胡10g,炙甘草6g;呕吐次数多加旋覆花、代赭石各10g),1剂/d,水煎300m L,早晚口服;西药治疗同对照组。连续治疗14d为1疗程。观测临床症状、椎动脉血流速度、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效26例,好转7例,无效1例,总有效率97.06%;对照组显效20例,好转7例,无效8例,总有效率77.14%;治疗组疗效优于对照组(P<0.05)。椎动脉血流速度两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]半夏白术天麻汤联合西药治疗痰浊中阻椎-基底动脉供血不足性眩晕,疗效满意,无严重不良反应,值得推广。[Objective] To observe the pinellia atractylodes rhizoma gastrodiae decoction combined western medicine in the treatment of phlegm turbidity resistance of vertebral-basilar artery insufficiency vertigo. [Methods] Using random parallel control method, 69 cases of hospitalized patients randomly divided into two groups according to clinic number order. The control group of 35 cases of vinpocetine 30mg＋0.9% sodium chloride 250 ml, static, 1 time/d; him horizon, 40 mg/time, 3 times/d, oral. Treatment group （34 cases of pinellia atractylodes rhizoma gastrodiae decoction （15g, ginger 10g pinellia, fried atractylodes rhizoma gastrodiae 12g, poria cocos, 20g dried tangerine or orange peel 15g, 10g angelica sinensis, rhizoma ligustici walliehii 15g, 18g dangshen, bupleurum, 10g, 6g main licorice; vomiting times more inula flower, red ochre 10g）, 1/d, Fried 300 mL water, morning and evening oral; western medicine treatment with the control group. Continuous treatment of 14 d to 1 course of treatment. Observation of clinical symptoms, TCD changes in blood flow velocity, adverse reactions. As a course treatment, judge curative effect. [Result] The treatment group （26 cases had marked effect, improvement in 7 cases, 1 had no effect, the total effective rate was 97.06%; control group 20 cases had marked effect, improvement in 7 cases, 8 no effect, the total effective rate was 77.14%; the treatment group curative effect is better than that of control group （P〈0.05）. TCD blood treatment group to improve the speed of change is better than that of control group （P〈0.01）. [Conclusion] Pinellia atractylodes rhizoma gastrodiae decoction combined western medicine in the treatment of phlegm turbidity resistance of vertebral - basilarartery insufficiency vertigo, curative effect is satisfied, no serious adverse reactions, worthy of promotion.
