神阙八阵穴艾灸配合中药灌肠治疗溃疡性结肠炎的随机对照研究

摘　　要：目的:观察神阙八阵穴艾灸配合中药灌肠治疗溃疡性结肠炎的临床疗效及对炎症细胞因子的影响。方法:选择符合纳入标准的溃疡性结肠炎住院患者135例,采用简单数字的随机方法随机分为治疗组、对照1组和对照2组,每组45例。对照1组给药口服美沙拉秦肠溶片治疗,对照2组在对照1组的基础上给予中药灌肠治疗,治疗组在对照1组的基础上给予神阙八阵穴热敏灸配合中药灌肠治疗。观察3组患者治疗前后细胞因子水平、症状、大肠黏膜积分变化,统计临床疗效。结果:治疗后治疗组与对照1组,对照2组在细胞因子(Hs-CRP、IL-6、TNF-α)水平、症候积分(腹痛、腹泻、脓血便)、大肠黏膜方面比较有显著性差异(P<0.05);治疗组、对照1组,对照2组临床疗效总有效率分别为93.33%、80.22%、82.22%,治疗组分别与对照1组,对照2组比较差异有统计学意义(P<0.05)。结论:神阙八阵穴热敏灸配合中药灌肠能够有效地缓解溃疡性结肠炎患者临床症状及大肠黏膜病变,改善细胞因子水平,值得进一步推广及应用。Objective： To investigate the clinical efficacy of Shenque eight array point moxibustion combined with traditional Chinese medicine enema for the treatment of ulcerative colitis and the influence on inflammatory cytokines. Methods： 135 patients with ulcerative colitis who met the inclusion criteria were selected and randomly divided into treatment group, controlled group 1 and control group 2, 45 cases were allocated in each group. The controlled group 1 was administered orally 1 Mesalazine Enteric- coated Tablets lg, 3 times a day, the controlled group 2 was given Chinese herbal enema on the basis of the controlled group 1, the treatment group was given Shenque eight array point moxibustion combined with traditional Chinese medicine enema on the ba- sis of the controlled group 1. Observing the 3 groups of patients before and after treatment on eytokine levels, symptoms, changes in colorectal mueosal score, statistical clinical efficacy. Results： After treatment, the treatment group and the controlled group 1 and controlled group 2 on cytokines （Hs-CRP, IL-6, TNF- alpha） level, symptoms （abdominal pain, diarrhea, pus and blood stool）, eolorectal mueosa had significant difference among them （ P 〈 0. 05 ） ; the treatment group and the controlled group 1, the controlled group 2 of the total clinical effective rate respectively were 93. 33% , 80. 22%, 82. 22%. The treatment group and the controlled group 1, controlled group 2, the difference was statistically significant （P 〈 0. 05 ）. Conclusion ： Shenque eight ar- ray points thermal moxibustion combined with traditional Chinese medicine enema can effectively alleviate the clinical symptoms of ulcerative colitis and colorectal mueosa in patients with lesions, improve the level of cytokines, it is worthy of further promotion and application.
