天牖五部分刺法联合推拿手法治疗小儿早期先天性肌性斜颈的临床观察

摘　　要：目的:观察天牖五部分刺法联合推拿手法对小儿早期先天性肌性斜颈的临床疗效。方法:将70例12月龄以内患儿随机分为治疗组与对照组,每组45例,对照组给予传统推拿手法治疗;治疗组在对照组的基础上给予天牖五部分刺法治疗。治疗后观察颈部旋转受限、颈部侧屈受限、颅面部不对称、胸锁乳突肌(SCM)肿块、胸锁乳突肌肌肉紧张消退情况,统计两组患者的临床疗效。结果:两组优良率差异无统计学意义(P>0.05)。在颈部被动活动度改善、颅面部对称、胸锁乳突肌肿块消退方面治疗组均优于对照组,差异有统计学意义(P<0.05)。治疗组与对照组临床有效率分别为91.43%、77.14%,比较有统计学意义(P<0.05)。结论:天牖五部分刺法联合推拿手法治疗先天性肌性斜颈早期疗效优于单纯推拿手法,为临床治疗小儿早期先天性肌性斜颈提供了有效的治疗方案。Objective： To observe the clinical effect of acupuncture combined with Tianyou five part massage for children with early congenital muscular tortieollis. Methods ： 70 cases of infants less than 12 months were randomly divided into treatment group and controlled group, 45 cases in each group, the controlled group was treated with traditional massage therapy, the treat- ment group was given Tianyou five part acupuncture on the basis of the controlled group. After treatment, neck rotation limita- tion, neck lateral flexion limitation, craniofacial asymmetry, sternoeleidomastoid muscle （SCM） mass, and sternocleidomastoid muscle tension subsided, and the clinical efficacy of the two groups were statistically analyzed. Results ： There was no significant difference between the two groups （ P 〉 0. 05 ）. In the neck passive activity improvement, craniofacial symmetry, sternoeleido- mastoid muscle mass subsided in the treatment group was better than the controlled group, the difference was statistically signifi- cant （P 〈 0. 05）. The clinical effective rate of the treatment group and the controlled group respectively were 91. 43% and 77. 14%, and the difference was statistically significant （ P 〈 0.05 ）. Conclusion ： Tianyou five part acupuncture combined with mas- sage for treating early stage of congenital muscular torticollis is superior to that of simple manipulation, provides the effective treat- ment for early clinical treatment of children with congenital muscular torticollis.
