布加综合征患者复发影响因素研究

摘　　要：目的描述布加综合征（BCS）患者流行病学特征，分析影响BCS患者复发的相关因素。为临床医师针对患者复发情况的治疗提供参考，改善患者预后情况，提高患者生命质量。方法纳入徐州医科大学附属医院介入放射科2008年1月-2012年12月收治的BCS住院患者488例，通过查阅相关病历资料，并进行随访调查，了解其预后复发情况。结果中位随访时间为42．75个月，488例BCS患者中复发158例，1、3、5、7年累积复发率分别为11％、26％、36％、42％。单因素分析得出：性别、年龄、腹腔积液、肝肿大、消化道出血、白细胞计数、血红蛋白、凝血酶原时间、国际标准化比值等因素在复发和未复发患者间差异有统计学意义（P〈0．05），多因素分析得出：性别、年龄、血尿素、乳酸脱氢酶、糖类抗原-125、诊断类型在复发和未复发患者间差异有统计学意义（P〈0．05）。结论男性、年龄≤30岁、血尿素偏低、乳酸脱氢酶升高、糖类抗原-125升高、肝静脉和混合型的布加综合征患者更易复发。Objective To describe the epidemiological characteristics of patients with Budd Chiari syndrome and to analyze the factors influencing recurrence in those patients. Methods A total of 488 Budd Chiari syndrome patients who were admitted into our department from January 2008 to December 2012 were selected. They were follow - up visited and their clinical data were collected for investigating prognosis. Results The median follow - up time was 42.75 months, and 158 out of 488 patients were relapsed. The 1, 3, 5 and 7 year cumulative recurrence rate was 11% , 26% , 36% , and 42%, respectively. According to single factor analysis, gender, age, peritoneal effusion, hepatomegaly, gastrointes- tinal bleeding, white blood cell count, hemoglobin, prothrombin time, and international normalized ratio were statistically different between the recurrent and non - recurrent patients （ P 〈 0.05 ）. Based on the multivariate analysis, gender, age, serum urea, lactic dehydrogenase, carbohydrate antigen 125 （CA125）, and the type of diagnosis were statistical different between recurrence and non - recurrence patients （ P 〈 0.05 ）. Conclusions Patients are more susceptible to Budd Chiari syndrome when they are men, less than 30 year old with a decreased blood urea level, increases in the a- mount of lactate dehydrogenase and CA125, and suffer with hepatic veins type or the mixed type of the condition.
