经脐自制单孔腔镜与三孔腹镜阑尾切除手术的比较

摘　　要：目的探讨经脐自制单孔腹腔镜阑尾切除术的有效性和安全性。方法采用随机数字表法，将我院就诊的60例急性阑尾炎患者随机分为对照组（给予三孔腹腔镜阑尾切除术治疗）与观察组（给予经脐自制单孔腹腔镜阑尾切除术），比较2组手术效果。结果观察组患者在住院时间、住院费用、术后疼痛、肛门排气及术后美容满意度等方面显著优于对照组（P〈0．05）；但观察组的手术时间明显较对照组延长（P〈0．05）；2组患者在术中出血量及并发症的发生方面差异无统计学意义（P〉0．05）。结论阑尾炎切除术中选择经脐自制单孔腔镜手术方式安全可靠，有利于患者的快速康复，改善美容效果，值得在临床广泛开展。Objective To explore the efficacy and safety of laparoscopic appendectomy using a transumbilical self - made single - hole endoscope. Methods A total of 90 patients with acute appendicitis who were admitted in our hospital from January 2016 to November 2016 were randomly divided into two groups ： a control group （ who underwent three - hole laparoscopic appendectomy） and an observation group （who received transumbilical self- made single -hole laparoscopie appendectomy）. Then, both groups were compared for surgical efficacy. Results Compared with the control group, improvements were found in the observation group as to hospitalization stay and cost, postoperative pain, anal ex- hausting and postoperative cosmetic satisfaction （ P 〈 0.05 ）, with remarkable extended operation time （ P 〈 0.05 ）. There was no significant difference in blood loss and complications between the two groups （P 〉 0.05 ）. Conclusions Appendectomy can be carried out using a transmbilical self - made single - hole endoscope, which is safe and reliable, and beneficial for rapid rehabilitation and can improve cosmetic effects. It is worthy to be widely applied in clinical practice.
