妊娠期孕妇身体质量指数增加率对剖宫产麻醉风险相关分析

摘　　要：目的研究妊娠期孕妇身体质量指数(BMI)变化率对剖宫产麻醉风险的相关性。方法 400例择期剖宫产实施椎管类麻醉患者,均计算孕期BMI增加率,然后通过采用等级资料的Logistic回归分析麻醉手术中低血压、术中寒颤、术中恶心呕吐、妊娠期高血压、妊娠期糖尿病、妊娠子痫、妊娠合并心力衰竭的发生与孕期BMI增加率的关系。结果采用等级资料的logistic回归分析结果显示,BMI增加率越高,发生术中低血压、术中寒颤、术中恶心呕吐、妊娠期高血压、妊娠合并心力衰竭的风险越高(B=0.79、0.71、0.69、0.95、0.91,P<0.05)。同时发现,BMI增加率与妊娠期糖尿病、妊娠子痫无关(P>0.05)。结论妊娠期孕妇BMI变化率与术中低血压、术中寒颤、术中恶心呕吐、妊娠期高血压、妊娠合并心力衰竭有关。Objective To study the correlation between increasing rate of body mass index （BMI） and cesarean section anesthesia risk in pregnant women. Methods Pregnancy BMI increasing rate of 400 selective cesarean section patients with intraspinal anesthesia was calculated. Logistic regression analysis of rating data was made on correlation between occurrence of intraoperative hypotension, intraoperative chills, intraoperative nausea and vomiting, gestational hypertension, gestational diabetes, eclampsid gravidarum, pregnancy complicated with heart failure and pregnancy BMI increasing rate. Results Logistic regression analysis of rating data showed that the higher of BMI increasing rate, the higher risk of occurrence of intraoperative hypotension, intraoperative chills, intraoperative nausea and vomiting, gestational hypertension, pregnancy complicated with heart failure （B=0.79, 0.71, 0.69, 0.95, 0.91, P〈0.05）, and BMI increasing rate had no correlation with gestational diabetes and eclampsid gravidarum （P〉0.05）. Conclusion BMI changing rate of pregnant women is correlated with intraoperative hypotension, intraoperative chills, intraoperative nausea and vomiting, gestational hypertension and pregnancy complicated with heart failure.
