摘  要:目的采取回顾性病例对照的研究方法,观察不同类型Gn在行体外受精-胚胎移植(IVF-ET)助孕患者中的应用,探讨不同类型Gn对IVF患者获卵及胚胎结局的影响。方法对243名因单纯输卵管因素行IVF长方案助孕患者新鲜周期获卵及胚胎结局进行回顾性分析,其中单纯用HMG进行COH 32例(A组),使用HMG+普利康进行COH77例(B组),使用HMG+果纳芬进行COH 134例(C组),对比三组的获卵结果及胚胎情况。结果 A组费用明显低于B组与C组,差异有统计学意义(P<0.05),在花费的情况下A组与B,C两组有相近的合格卵率、受精率、优质胚胎率,但差异均无统计学意义(P>0.05);与A组C组相比,B组具有较高的2PN卵裂率,差异有统计学意义(P<0.05);与A组B组相比,C组Gn用量较明显减少、Gn费用较多、获卵个数较多,且差异有统计学意义(P<0.05)。三组患者在年龄、不孕年限、BMI、基础卵泡刺激素(bFSH)、基础黄体生成素(bLH)、基础雌二醇(bE_2)、Gn天数等方面无显著差异(P>0.05)。结论对于IVF患者,使用不同类型的Gn可对获卵及胚胎结局产生不同的影响。Objective: To observe the application of different types of Gn in vitro fertilization-embryo transfer (IVF-ET) assisted pregnancy by retrospective case-control study, to explore the effect by different types of Gn on ovulation and embryo outcome of IVF patients.Methods: A retrospective analysis was conducted on 243 ovarian and embryo outcomes in 243 patients with IVF-long protocol assisted by simple tubal factors. 32 cases (group A) was treated with HMG alone and group B (77 patients) treated with HMG +Pulicron, (Group C) were treated with HMG + Gnaphal, and the results of ovulation and embryo were compared between the three groups. Results: The cost of group A was significantly lower than that of group B and group C (P〈0.05) . In group A, the cleavage rate of 2PN and transplantable embryo between group A with group C was similar to those in group B (P〉 0.05) . Compared with group A and group C, the cleavage rate of 2PN in group B was higher, and the difference was significant (P〈0.05) . Compared with group A and group B, the amount of Gn in group C was significantly decreased, the cost of Gn was higher, the number of ovum was higher, and the difference was statistically significant (P〈0.05) . There were no significant differences in age, infertility, BMI, basal follicle stimulating hormone (bFSH) , basal luteinizing hormone (bLH) , basal estradiol (bE2) and Gn days in the three groups (P〈0.05) . Conclusions: Different types of Gn may have different effects on ovulation and embryo outcome in IVF patients.
