单纯心内修复在法洛四联症矫治中的应用

摘　　要：目的 总结用单纯心内修复的方法施行法洛四联症矫治的手术经验.方法 2005年1月到2016年7月武汉亚洲心脏病医院对275例法洛四联症患者施行单纯心内修复,在充分疏通右室流出道、修补室间隔缺损后直接缝合右室切口,占同期法洛四联症矫治术的28.61％.结果 全组无围术期死亡.所有病例术后血流动力学较平稳,术后恢复良好,无重大并发症.结论 法洛四联症的矫治宜选择个性化的手术方案,对单纯漏斗部狭窄、室间隔缺损为嵴下型的病例,单纯心内修复方法可行,并有利于心功能保护及术后恢复.Objective To summarize the experience of using simple cardiac repair to retreat Tetralogy of Fallot （TOF）.Methods 961 patients underwent total correction of tetralogy of Fallot in our hospital between January 2005 to July 2016.275 cases,accounting for 28.61%,received operation by means of simple cardiac repair.When right ventricular outflow tract （RVOT） obstruction relieved and VSD repaired,incision of RV was closed directly.Results The peak systolic pressure gradient from right ventricle to pulmonary artery in all patients was under 30 mm Hg.No patients died in hospital,without low cardiac output syndrome,renal failure or other severe complications.Postoperative ventilation time was （12.29＆#177;8.67）hours,intensive care unit stay was （35.10＆#177;19.80） hours,and hospital stay was （9.12＆#177;5.20）days.Conclusion Simple cardiac repair with no patch to enlargement RVOT may have excellent results in those TOF patients with only infundibular stenosis and infracristal VSD.This method is advantageous to preserve right ventricle function.
