窒息新生儿生后早期振幅整合脑电图特点研究

摘  要:目的研究探讨窒息新生儿生后早期振幅整合脑电图(aEEG)的临床表现及特点。方法 50例窒息新生儿作为观察组,同期50例健康新生儿作为对照组,均在出生后的6 h内对其进行早期aEEG监测,观察其背景活动、睡眠-觉醒周期(SWC)以及痫性活动特点,并对各指标与患儿窒息程度、脑损伤程度之间的相互关系进行分析。结果观察组新生儿aEEG监测结果中背景活动异常率为42.0%、无SWC率为12.0%、痫性活动发作率为20.0%显著高于对照组的0、0、0,且有重度窒息新生儿同时高于轻度窒息新生儿的情况,差异有统计学意义(P<0.05)。统计不同严重程度的缺血缺氧性脑病新生儿的aEEG表现,可见轻、中、重度患儿比较,差异有统计学意义(P<0.05)。结论通过早期aEEG可以对窒息新生儿的脑损伤情况进行及时诊断,为临床治疗干预提供依据,具有临床推广和应用价值。Objective To study and explore the clinical manifestation and features of early amplitude- integrated electroencephalogram (aEEG) of postnatal asphyxia newborns. Methods There were 50 asphyxia newborns as observation group, and concurrent 50 healthy newborns as control group. Monitor was made on early aEEG of newborns in postnatal 6 h, so as to observe their background activity, sleep-waking cycle (SWC) and epileptiform activity features and analyze the mutual relation of all indexes and asphyxia degree, cerebral injury degree. Results According to aEEG monitoring results, the observation group had abnormal rate background activity as 42.0%, non SWC rate as 12.0% and attack rate of epileptiform activity as 20.0%, which were all significantly higher than 0, 0 and 0 in the control group, and situation of severe asphyxia newborns was higher than mild asphyxia newborns. Their difference had statistical significance (P〈0.05). Adding up aEEG performance of newborns with hypoxic-ischemic encephalopathy in different severity showed that mild, moderate and severe patients had statistically significant difference (P〈0.05). Conclusion Early aEEG can timely diagnose cerebral injury situation, and provide basis for clinical treatment intervention. So it has clinical promotion and application value.
