耳内镜下鼓膜置管联合腺样体切除术治疗儿童分泌性中耳炎临床疗效分析

摘　　要：目的对耳内镜下鼓膜置管联合腺样体切除术治疗儿童分泌性中耳炎临床疗效进行分析。方法 50例儿童分泌性中耳炎患儿随机分为A组与B组,各25例。A组采用单纯耳内镜下鼓膜置管治疗,B组采用耳内镜下鼓膜置管联合腺样体切除术治疗。比较两组患儿分泌性中耳炎治疗总有效率;积液持续存在时间、耳鸣耳闷消失时间、听力恢复正常时间、鼓膜愈合时间;治疗前后患儿听力相关指标的差异。结果 B组患儿治疗总有效率为96.00%,明显高于A组的76.00%,差异具有统计学意义(P<0.05)。治疗前两组听力相关指标比较差异无统计学意义(P>0.05)。治疗后两组听力相关指标均明显改善,且B组改善幅度更大,差异具有统计学意义(P<0.05)。B组患儿积液持续存在时间、耳鸣耳闷消失时间、听力恢复正常时间、鼓膜愈合时间均明显短于A组,差异具有统计学意义(P<0.05)。结论耳内镜下鼓膜置管联合腺样体切除术治疗儿童分泌性中耳炎临床疗效确切,可有效促进患儿临床症状消退,改善患儿听力水平,值得推广。Objective To analyze clinical effect by otoscopic grommet insertion combined with adenoidectomy in the treatment of children secretory otitis media. Methods A total of children patients with secretory otitis media were randomly divided into group A and group B, with 25 cases in each group. Group A received single otoscopic grommet insertion for treatment, and group B received otoscopic grommet insertion combined with adenoidectomy for treatment. Comparison was made on total effective rate in secretory otitis media treatment, hydrops existence time, tinnitus and aural fullness disappearance time, hearing recovery time, tympanal healing time and hearing-related indexes before and after treatment between the two groups. Results Group A had obviously higher total effective rate as 96.00% than 76.00% in group A, and their difference had statistical significance （P〈0.05）. There was no statistically significant difference of hearing-related indexes before treatment between the two groups （P〉0.05）. After treatment, both groups had obviously improved hearing-related indexes, and group B had larger improvement, and the difference had statistical significance （P〈0.05）. Group B had all shorter hydrops existence time, tinnitus and aural fullness disappearance time, hearing recovery time, and tympanal healing time than group A, and their difference had statistical significance （P〈0.05）. Conclusion Combination of otoscopic grommet insertion and adenoidectomy shoes precise curative effect in treating children secretory otitis media. It can effectively accelerate clinical symptom resolving and improve hearing in children. This method is worth promoting.
