腰大池引流在自发性小脑出血并破入脑室患者中的应用

摘　　要：目的探讨腰大池引流在自发性小脑出血并破入脑室患者中的应用效果。方法选取我院2010年1月~2016年3月收治的90例自发性小脑出血破入脑室患者进行研究,按随机数字表法将其分为研究组和对照组,对照组采用颅内血肿微创清除术治疗,研究组在此基础上增加腰大池引流,治疗结束后比较两组患者近期、远期疗效及感染情况。结果研究组近期疗效优于对照组,差异均有统计学意义(Uc=2.035>u,P<0.05)。研究组远期疗效优于对照组,差异均有统计学意义(Uc=2.014>u,P<0.05)。治疗期间无严重感染病例发生,两组患者共发生发热症状者21例,其中研究组12例(26.67%),对照组9例(20.00%),经脑脊液培养均证实为阴性,发热症状发生率比较差异无统计学意义(χ~2=0.559,P>0.05)。结论在颅内血肿微创清除术的基础上增加腰大池引流治疗,可改善小脑出血并破入脑室治疗效果,生存质量较好。Objective To investigate the clinical effects of lumbar cistern drainage in patients with cerebellar hemorrhage and broken into ventricle. Methods 90 cases of patients with cerebellar hemorrhage and broken into ventricle in our hospital from January 2010 to March 2016 were chosen to research, and were divided into research group and control group, control group were treated with minimally invasive surgery for intracranial hematoma, research group were treated withlumbar cistern drainage on the basis of control group,short-term efficacy, long-term efficacy and infection between two group were compared. Results Short-term efficacy of research group were better than that of control group,the difference was statistically significant （Uc=2.035〉u,P〈0.05）. Long-term efficacy of research group were better than that of control group,the difference was statistically significant （Uc=2.014〉u,P〈0.05）. No serious infection occurred during the treatment,23 cases of patients with fever symptoms occurred in two groups, 12 cases in research group, 9 cases in control group, and were confirmed to be negative by cerebrospinal fluid culture, there were no significant differences in fever symptoms between two group （X^2=0.559, P〉0.05）. Conclusion Lumbar cistern drainage combined with minimally invasive surgery for intracranial hematoma could improve the clinical effects of patients with cerebellar hemorrhage and broken into ventricle,and with good life quality.
