社区老年人脑卒中相关危险因素及其干预效果

摘　　要：目的探讨社区老年人群脑卒中相关危险因素及其干预效果。方法选取2015年1月至2016年12月临湘市人民医院管辖的所有社区医院的5 412例年龄60~70岁常驻居民作为研究对象,对其进行问卷调查,并给予健康指导,分析脑卒中相关危险因素改善情况。结果存在至少一项脑卒中相关危险因素者5 260例,占97.2%,健康指导干预后,达标者281例,占5.3%。logistic分析发现,女性、吸烟、肥胖与干预有效性呈负相关(校正OR=-1.603、-8.851、-4.725,P<0.05)。周围血管史、运动缺乏、收缩压、血糖、三酰甘油、低密度脂蛋白与干预有效性呈正相关(校正OR=0.591、0.641、0.961、0.778、0.551、0.481,P<0.05)。结论对社区老年人群进行筛查和干预,可在一定程度上改善脑卒中危险因素控制,为脑卒中提供一种新的防控模式。Objective To investigate the risk factors of stroke in community elderly and the effect of intervention measures. Methods A total of 5 412 permanent residents with 60-70 years old in all the community hospitals under the jurisdiction of People＇s Hospital of Linxiang City from January 2015 to December 2016 were selected as the research object, they were given a questionnaire and health guidances, improvement of related to risk factors of stroke was analysed. Results There were at least one related to risk factors of stroke in 5 260 cases, accounting for 97.2%, after the intervention of health guidelines, the compliance of the patients in 281 cases, accounting for 5.2%. Logistic analysis found that the effect of the intervention were negatively related to women, smoking, obesity （the correction, OR =-1.603,-8.851,-4.725,P〈0.05）; the effect of the intervention were positively related to peripheral vascular history, lack of exercise, systolic blood pressure, blood glucose, triglyceride,low density lipoprotein （the correction 0R=0.591,0.641,0.961,0.778,0.551,0.481 ,P〈0.05）. Conclusion Screening and intervention for the commu- nity elderly has a certain improvement on the risk factors of stroke,which provides a new model for prevention and control of stroke.
