摘　　要：患者女,72岁,左小腿"糖尿病足"截肢术后丘疹、结节、溃疡伴痒半年,多次按湿疹、痒疹治疗无效。皮损伴明显瘙痒。半月前,外用药物后病情加重,皮损蔓延至对侧。结节组织病理提示:表皮局部呈杯状下陷,其内为坏死物质,下方为坏死变性的胶原,有真皮胶原纤维穿通现象,真皮浅层血管周围炎。Verhoeff-Van Gieson染色提示胶原纤维有穿通现象。诊断:反应性穿通性胶原病。A 72-year-old female, complaining of itch ,had been suffered with papule, nodule and ulcer on the left lower leg after diabetic foot amputation for half a year, repeatedly misdiagnosed as eczema and prurigo, but the symptom did not get better. She felt very itch. Half a month ago, the lesions spread to the fight side lower leg after using of poly-iodine. Histological examination showed cup-shaped depression containing parakeratotic cells intermin- gled with basophilic material and altered collagen bundles. Verhoeff-Van Gieson stain revealed vertically orien- ted collagen fibers in upper dermis, ready to initiate formation of the perforating channels. The diagnosis of re- active perforating collagenosis was made,
