玻璃体腔内注射雷珠单抗联合七叶皂苷钠治疗糖尿病性黄斑性水肿疗效观察

摘　　要：目的观察玻璃体腔内注射雷珠单抗联合七叶皂苷钠治疗糖尿病性黄斑性水肿(DME)的临床疗效。方法将102例DME患者(135眼)按照随机数字表分为观察组(51例,67眼)和对照组(51例,68眼),对照组接受玻璃体腔内注射雷珠单抗治疗,每月1次,连续治疗6次;观察组在对照组治疗基础上加用七叶皂苷钠片治疗,2次/d,60 mg/次,疗程为6个月。观察2组治疗前后眼压、视力、黄斑厚度以及黄斑区渗漏情况,统计2组临床疗效。结果2组治疗后眼压均有一定程度的降低,观察组治疗后3个月、6个月的眼压均显著低于对照组(P均<0.05);观察组治疗后2个月、3个月、6个月的最佳矫正视力均显著高于对照组(P均<0.05),黄斑中心凹厚度改善情况均优于对照组(P均<0.05),黄斑区发生荧光渗漏的比例显著低于对照组(P均<0.05);观察组治疗后黄斑水肿治疗效果、视力提高率均优于对照组(P均<0.05)。2组治疗期间均未出现严重的药物不良反应。结论玻璃体腔内注射雷珠单抗联合七叶皂苷钠治疗DME能够显著改善患者的黄斑水肿及视网膜渗漏,稳定现存视力,巩固疗效,促进患者康复,且安全性高,值得临床推荐。Objective It is to observe the clinical efficacy of intravitreal injection of monoclond antibody combined with seven sodium aescinate in the treatment of diabetic macular edema （DME）. Methods 102 DME patients （135 eyes） were randomly divided into observation group （cases, 67 eyes） and control group （5! cases, 68 eyes） , the patients in the control group were treated with intravitreal injections of monoclonal antibody, once a month, and 6 times for continuous treatment; and the patients in the observation group were treated with sodium aescinate tablets on the basis of the treatment of the control group, twice per day, 60rag each time, and the course of treatment was 6 months. The indicators of intraocular pressure （IOP） , visual acuity, macular thickness and macular leakage were observed before and after treatment in the 2 groups, and the clinical efficacy of the 2 groups was statistically analyzed. Results The IOP of the 2 groups were decreased to some extent after treatment, the IOP at 3 months and 6 months after treatment in the observation group was significantly lower than those in the control group （ all P 〈 O. 05） ; after treatment for 2 months, 3 months and 6 months, the best corrected visual acuity of the observation group was significantly higher （ P 〈 0.05） , the macular thickness was significantly better （ P 〈 0.05 ） , and the oc- currence of macular fluorescein leakage ratio was significantly lower than the control group （ P 〈 0.05 ）. The therapeutic effect and visual acuity rate of macular edema after treatment in observation group were better than those in the control group （ all P 〈 0.05 ）. There was no serious adverse drug reaction during treatment in the 2 groups. Conclusion Intravitreal injection of mon- oclonal antibody combined with seven sodium aescinate in treatment of DME can significantly improve macular edema and reti- nal leakage in patients, stabilize existing eyesight, consolidate curative effect, promote the rehabilitation of the pati
