口服中药托里透脓汤加减联合闭式滴注+负压引流技术VSD治疗难治性褥疮的临床疗效

摘　　要：目的为了进一步促进难治性褥疮的愈合效果,研究和探讨应用口服中药托里透脓汤加减联合闭式滴注+负压引流技术(VSD)的价值和意义。方法根据患者住院号先后顺序从2015年4月~2016年3月期间来我院就诊的难治性褥疮患者中选取24例作为研究对象并通过随机、双盲、对照的原则将其分为试验组和对照组各12例,其中接受传统治疗措施的设为对照组,接受口服中药托里透脓汤加减联合闭式滴注+负压引流技术(VSD)方案治疗的设为试验组,探讨不同治疗方案对两组疗效影响差异。结果研究表明,试验组和对照组都能够获得一定的治疗效果,但和对照组相比试验组患者的褥疮严重程度指数(SSI)评分明显降低,且试验组的清创平均次数、治疗平均时间以及正常体温恢复时间明显减少,两组比较差异具有统计学意义(P<0.05)。结论难治性褥疮患者选取口服中药托里透脓汤加减联合闭式滴注+负压引流技术(VSD)方案治疗,有利于促进褥疮创面愈合,缩短治疗时间和清创次数,减轻患者痛苦,效果显著,值得借鉴推广。Objective In order to further promote the healing effect of refractory bedsore, to study and discuss the value and significance of oral administration of Tori decoction combined with closed infusion ＋ negative p,ressure drainage （VSD）. Methods According to patient＇s order of hospitalization, 24 patients with refractory decubitus ulcers in our hospital from April 2015 to March 2016 were selected as the subjects. Through randomized, double-blind, controlled principle, they were divided into experimental group and control group, 12 cases in each group. Patients treated with traditional treatment measures were as the control group, and patients in treatment group were treated with oral Chinese medicine Tori and pus soup plus and minus closed infusion ＋ negative pressure, drainage （VSD）.The effects, of different treatment regimens on the two groups was investigated. Results The results showed that the experimental group and the control group could achieve certain therapeutic effect, but compared with the control group, the bedsore severity index （SSI） score of the experimental group was significantly lower, and average number of debridement, the average treatment time and the normal temperature recovery time of the experimental group were significantly reduced, the difference between the two groups was statistically significant （P 〈0.05）. Conclusion Qral Chinese medicine Tori and Tang Yah decoction combined with closed infusion ＋ negative pressure drainage （VSD） program in treatment of refractory bedsore patients is beneficial to promote the healing of bedsore, shorten the time of treatment and the number of debridement, and relieve the pain of patients. It has remarkable effect, and it is worthy of reference and promotion.
