摘 要:目的探讨血清和尿液的微小RNA-29c(miR-29c)表达水平在红斑狼疮性肾炎(LN)患者早期肾纤维化中的临床意义。方法对60例LN早期肾纤维化患者(LN组)行肾穿刺活检并依据肾脏纤维化慢性指数(CI)将其分为高、中、低CI3组,选取同期15例健康志愿者(对照组)和15例非狼疮性慢性肾脏病(CKD)患者(非狼疮性CKD组)作为对照。分别检测所有研究对象的补体c3、c4水平、抗双链DNA(dsDNA)抗体浓度、24小时尿蛋白、血尿素氮(BUN)、血清肌酐(Cr)水平以及肾小球滤过率估计值(eGFR),采用实时定量聚合酶链反应(qRT—PCR)法检测血清和尿液中的miR-29c相对表达量。结果LN组、非狼疮性CKD组中血清miR-29c的表达水平均明显低于对照组(P〈0.05),而且LN组患者血清中miR-29c的表达明显低于非狼疮性CKD组。LN组患者尿液中的miR-29c表达量明显低于非狼疮性CKD患者(P〈0.05)。LN组患者尿液与血清miR-29c的表达量呈负相关,而两者在非狼疮性CKD患者中呈正相关。血清和尿液中miR-29c的表达均与CI评分呈正相关,而且高、中、低CI组血清miR-29c水平均高于尿液(P〈0.05)。血清和尿液中miR-29c与免疫类和。肾功能指标均存在相关性。结论miR-29c总体上与肾脏的慢性纤维化程度以及。肾功能存在相关性,表明miR-29c可作为LN患者肾脏纤维化早期进展的一种新型生物标记物。Objective To assess the clinical significance of serum and urine miR-29c expression in early renal fibrosis in lupus nephritis (LN). Methods All eligible 60 LN cases with early renal fibrosis were collected as experimental group( LN group). These patients were performed by renal biopsies and divided into high ehrouicity index(CI) group,modium CI group and low CI group according to renal CI. laboratory indexes detected such as level of alexin C3 and CA,anti-double-stranded DNA(anti-dsDNA) ,24h proteinuria, serum creatinine, estimated glomerular filtration ratio ( eGFR ) , and blood urea nitrogen ( BUN ). The detection of serum and urine miR-29c expression was performed using qRT-PCR. Results The relative expression of serum miR-29c was lower in LN and non-LN CKD patients than healthy controls( P 〈 0.05 ), while the level was lower in LN patients than non-LN patients ( P 〈 0.05 ). MiR-29c level of urine in LN was patients significantly lower than that of non-LN CKD patients( P 〈 0.05 ). Obvious correlation between serum and urine milR-29c in LN patients was positive, but negative correlation was found in non-LN patient. The level of miR-29c in urine and serum was positively associated with CI grade while of miR-29c the level was higher in serum than in urine ( P 〈 0.05 ). Besides, miR-29c in serum and urine was highly associated with other laboratory indexes such as immune and renal function. Conclusion MiR-29c is associated with the degree of chronic renal fibrosis and renal function, revealing that it can be served as a novel serum or urine marker for early progression of fibrosis in LN patients.
