配合型输血与同型输血对患者红细胞不规则抗体产生的影响

摘　　要：目的分析配合型输血与同型输血对患者红细胞不规则抗体产生的影响。方法回顾性分析本院2014年5月—2016年5月输血治疗的400例患者临床资料,将因特殊情况实施ABO、Rh D配合型输血的200例患者设为观察组,将同型输血的200例患者设为对照组,比较2组输血后对红细胞不规则抗体产生的影响。结果观察组红细胞不规则抗体阳性检出率4.5%、不良输血反应发生率3%,同期对照组红细胞不规则抗体阳性检出率7%、不良输血反应发生率4%,差异无统计学意义(P>0.05);2组输血有效性比较差异无统计学意义(P>0.05)。结论同等有效性的前提下配合型输血红细胞不规则抗体阳性检出率及不良输血反应发生率相较于同型输血更低,为当前临床输血治疗方案的最佳选择。Objective To explore the effect of matched blood transfusion and the same type of blood transfusion on production of irregular erythrocyte antibodies of patients. Methods The clinical data of 400 patients with the blood transfusion therapy were collected and analyzed retrospectively from May 2014 to May 2016, and 200 patients who accepted the ABO Rh D matched blood transfusiondue to special circumstances were selected as an observing group, and 200 patients who received the same type of blood transfusion were selected as a control group. The effectsof the matched blood transfusion and the same type of blood transfusion on the production of irregular erythrocyte antibodies of the patientswereobserved and the results were compared between the two groups. Results Thepositive rate of irregular erythrocyteantibody was4.5% and the adverse transfusion reaction rate was 3% in the observation group, and the positive rate of irregular erythrocyte antibody was 7.0% and the adverse transfusion reaction rate was4.0% in the control group, and there were no significant differences between the two groups（P〉0.05）.In addition, there was no significant difference between the two groups in the blood transfusion effectiveness（P〉0.05）. Conclusion With the premise of equal effectiveness,the positive rate of irregular erythrocyte antibody and the adverse transfusion reaction rateare lower in the matched blood transfusion group,compared with those in the same type of blood transfusion group, which gives the best choice for the current clinical blood transfusion therapy.
