摘　　要：目的了解三甲综合医院住院患者因呼吸机相关肺炎(VAP)和颅脑手术部位感染(NSSI)导致的平均住院日延长和直接经济损失,为制定预防和控制医院感染决策提供依据。方法采用1∶1匹配的病例对照研究设计,筛选江苏省4家三甲医院2013年5月-2014年4月期间住院患者进行回顾性分析。结果纳入研究对象共226对,其中VAP 155对,SSI 76对。因VAP导致患者平均住院费增加(10.43±12.24)万元,平均住院日延长(16.67±33.57)d。因SSI导致的平均住院费增加(2.90±3.18)万元,平均住院日延长(15.13±19.65)d(P<0.001)。结论VAP和NSSI可以显著增加患者的经济负担和疾病负担,需要采取积极有效的措施,预防与控制VAP和NSSI发生,保障患者安全。Objective To understand direct economic losses and disease burdens for Ventilator - Associated Pneumonia （VAP） and Neurosurgical Site Infection （NSSI） ,so as to provide the scientific basis for infection control and prevention. Methods We performed 1：1 matched case - control study design to explore the association. Results In a11,226 pairs of cases and controls were available in this study,including 155 pairs for VAP and 76 pairs for NSSI. We found that the aver- age cost of hospitalization increased 10.43 ＋ 12.24 thousand Yuan （P 〈 0. 001 ） and the average hospitalization duration of extended 16.67 days （P 〈 0. 001 ）for each VAP patient. Meanwhile, the average cost of hospitalization increased 2.90 ± 3.18 thousand Yuan （ P 〈 0.001 ） and the average hospitalization duration of extended 15.13 days （ P 〈 0. 001 ） for each NSSI. Conclusion VAP and NSSI significantly increase the economic burden and disease burden for in - patient. So we must take active and effective measures to prevent and control the occurrence of VAP and NSSI for protecting patient safety.
