摘  要:目的探讨负压引流技术(VSD)联合高渗盐水灌洗在治疗闭合性潜行性皮肤剥脱损伤中的效果。方法回顾性分析该院2009年3月—2016年3月,收治的18例闭合性皮肤潜行性剥脱伤患者,均行手术切开引流创面刮除手术,术后应用VSD负压吸引并采用高渗盐水(10%NaCl)间断低速灌洗(1次/6h,20m/次)。结果 18例闭合性皮肤潜行性剥脱伤患者,均未发生明显引流管堵塞,拆除VSD海绵时,肉眼见创面肉芽生长良好,未见感染病灶,缝合后伤口均顺利愈合,患者均获随访,随访时间6~24个月,平均12个月。上述病例均未复发,肢体关节外观满意,活动度良好,血运及感觉正常。结论闭合性皮肤潜行性剥脱损伤应用VSD联合高渗盐水溶液间断灌洗治疗,可显著改善负压吸引效果,减少引流管堵塞,延长VSD使用时间,促进皮肤与深部筋膜组织粘连,缩短治疗时间,临床治疗效果满意。Objective To investigate the effectiveness of the vacuum sealing drainage (VSD) technique with hypertonic saline for the treatment of skin closed internal degloving injury(CIDI). Methods A retrospec- tive analysis of author'hospital in March 2009 to March 2016, there were 18 cases of (CIDI) patients under- went surgery incision and drainage of fhe wound scratch in operation, postoperative application VSD and hypertonic saline with 10~ NaCL discontinuous low- speed lavage (1 time/6h,20m/1 time). Results There was no significant drainage tube blockage in all 18 cases of (CIDI) patients. When removal of VSD sponge, the wound granulation growth were good by naked eyes to see, and no infectd wound healing after suture. All patients were followed up for 6 to 24 months with an average of 12 months. All of the above cases did not recur, the joints of the limbs were satisfied with their appearance, good activity, blood perfu- sion and sensation. Conclusion (CIDI)applieation of VSD combined with hypertonic saline solution lavage in the treatment of discontinuous can significantly improve the negative pressure attracted effect, reduce the blockage of the "drainage tube, prolong the service life of the VSD, promote skin and deep fascia tissue adhesive contact, shorten the treatment time. The therapeutic is effect in clinical.
