摘 要:目的探讨多层螺旋CT(MSCT)、X线平片诊断胫骨中下1/3螺旋形骨折合并后踝骨折的临床价值。方法选取2012年1月—2016年1月该院收治的194例骨中下1/3螺旋形骨折患者的术前X线平片、MCST检查资料进行回顾性分析,对比二种检查方法对胫骨中下1/3螺旋形骨折合并后踝骨折的检出率。结果 148例确诊后踝骨折患者中,X线检出47例,诊断胫骨中下1/3螺旋形骨折合并后踝骨折的灵敏度为31.76%、特异度为95.65%、漏诊率为68.24%、误诊率为4.35%;MSCT检出124例患者,诊断胫骨中下1/3螺旋形骨折合并后踝骨折的灵敏度为83.78%、特异度为100.00%、漏诊率为16.22%、误诊率为0.00%。结论 MSCT诊断胫骨中下1/3螺旋形骨折合并后踝骨折的灵敏度显著的优于X线平片。Objective To investigate the clinical value of multi- slice spiral CT (MSCT) and X- ray plain film in the diagnosis of 1/3 fracture of lower tibia and ankle fracture. Methods From January 2012 to Jari- uary 2016 in author's hospital, 194 cases of bone screw under 1/3 were fracture patients with preoperative X - ray and MCST examinations were retrospectively analyzed and compared two methods combined with an- kle fracture detection rate of fracture of lower tibial helical 1/3. Results Totally 148 cases were diagnosed after ankle fracture patients, X- ray diagnosis of 47 cases, the diagnosis of lower tibial 1/3 spiral fracture combined with ankle fracture sensitivity was 31.76%, specificity was 95.65%, the misdiagnosis rate was 68.24%, the misdiagnosis rate was 4.35% ; MSCT detected 124 cases of patients, in the diagnosis of tibia 1/3 spiral fracture combined with ankle fracture sensitivity was 83.78%, specificity was 100%, the misdi- agnosis rate was 16. 22%, the misdiagnosis rate was 0%. Conclusion The sensitivity of MSCT in the di- agnosis of 1/3 fracture of lower tibia and ankle fracture is significantly better than that of X - ray plain film.
