摘  要:目的探究不同机械通气模式对新生儿胎粪吸入综合征血流动力学的影响。方法选取2014年1月~2016年7月我院新生儿胎粪吸入综合征104例为研究对象,采用随机数字表法分为常规通气组和高频通气组,每组52例。常规通气组采用常频机械通气治疗方法 ,高频通气组采用高频振荡机械通气治疗方法。比较两组患儿治疗总有效率、呼吸机应用时间、住院时间;比较两组患儿治疗前后氧分压、二氧化碳分压、每搏量、搏动指数。结果高频通气组患儿新生儿胎粪吸入综合征治疗总有效率高于常规通气组(P<0.05);高频通气组呼吸机应用时间、住院时间均短于常规通气组(P<0.01);治疗前两组患儿氧分压、二氧化碳分压、每搏量、搏动指数相似(P>0.05);治疗后高频通气组氧分压、二氧化碳分压显著优于常规通气组(P<0.05)。治疗后两组每搏量、搏动指数与治疗前比较,差异无统计学意义(P>0.05)。结论高频机械通气模式对新生儿胎粪吸入综合征血流动力学有改善作用,可更好地改善血气分析结果 ,缩短呼吸机治疗时间和住院时间,改善患儿预后。Objective To explore the effect of different mechanical ventilation patterns on hemodynamics of neonatal meconium aspiration syndrome.Methods A total of 104 cases of neonatal meconium aspiration syndrome from January 2014 to July 2016 in our hospital were selected as the study object,they were divided into conventional ventilation group and high frequency ventilation group by random number table,each group of 52 cases. The conventional ventila- tion group was treated by constant frequency mechanical ventilation,the high frequency ventilation group was treated by high frequency oscillating mechanical ventilation.The effective rate,time of ventilator,hospitalization time were compared between the two groups;the pressure of oxygen,the partial pressure of carbon dioxide,the stroke volume and the pulse index before and after treatment were compared between the two groups.Results The total effective rate of the high fre- quency ventilation group was higher than that of the conventional ventilation group (P〈0.05).The time of ventilator,hos- pitalization time of the high frequency ventilation group were shorter than those of the conventional ventilation group (p〈0.01).The oxygen partial pressure,carbon dioxide partial pressure,stroke volume and pulse index were similar in the two groups before treatment (P〉0.05);the partial pressure of oxygen and the partial pressure of carbon dioxide in the high frequency ventilation group were significantly better than those in the conventional ventilation group (P〈0.05). There was no significant difference in stroke volume,pulsatility index and before treatment between the two groups (P〉0.05). Conclusion The high frequency mechanical ventilation model can improve the hemodynamics of neonatal meconium aspiration syndrome,improve the results of blood gas analysis,shorten the time of ventilator treatment and the time of hospitalization,and improve the prognosis of children.
