椎管内麻醉无痛分娩对母婴近期和远期的影响分析

摘　　要：目的探讨椎管内麻醉无痛分娩对母婴近期和远期的影响。方法本研究选取我院分娩的单胎头位足月的初产妇120例为研究对象,受试对象分为椎管内麻醉无痛分娩组和对照组,每组各60例;收集比较母亲产后出血、感染发生率、神经损伤发生率等为指标水平以及新生儿Apgar评分和随访的社会功能情况评分,并进行比较分析;采用SPSS 17.0统计学软件完成统计工作。结果两组产妇在产后出血的发生比例、感染以及产后尿潴留的发生比例均未见统计学差别;神经损伤的产妇在无痛分娩组见1例,发生率为1.67%;在对照组未见神经损伤者;此外,两组均未发现产后出现瘫痪情况。两组新生儿出生身长和体重未见差别,Apgar评分和脐血p H值水平也均表现正常,且未见差别;7岁时的AAMD-ABS总分在无痛分娩组为(192.34±13.45),对照组为(196.04±13.98),但未见统计学差别。结论椎管内麻醉用于无痛分娩,对母婴双方近期、远期均无不良影响,值得临床推广。Objective To investigate the short term and long term effects of painless labor with spinal anesthesia on puerperal and infant. Methods A total of 120 maternal births were randomly divided into two groups： control group （n = 60） and control group （n = 60）; （c） The incidence of neonatal Apgar score and follow-up social function scores were compared and analyzed. SPSS17.0 statistical software was used to complete the statistical work. Results There was no significant difference in the proportion of postpartum hemorrhage between the two groups, the incidence of postpartum hemorrhage and the incidence of postpartum urinary retention were not statistically significant. The incidence of postpartum hemorrhage was 1.67% （one case） in the painless delivery group. In the control group, there was no nerve injury case; In addition, the two groups were not found postpartum paralysis of the situation. There was no difference in body length and body weight between the two groups. Apgar score and umbilical cord blood pH were also normal and no difference. The total AAMD-ABS score at the age of 7 was 192.34± 13.45 in the painless delivery group Group was 196.04± 13.98, but no statistically significant difference.Conclusions Painless labor with spinal anesthesia has no harmful effect on puerperal and infant from the short term and long term sides, which is worthy of clinical promotion.
