摘  要:目的评估COPD患者甲状腺功能,并分析肺功能、动脉血气及甲状腺激素之间的关系。方法 70例COPD患者(分4级)及50例健康志愿者(对照组),分别测定其有关通气功能指标(FEV1、FVC、FEV1/FVC)和血气分析参数(PaCO_2、PaO_2、SO_2)以及血清中甲状腺素总三碘甲状腺原氨酸(TT_3)、总甲状腺素(TT_4)、游离三碘甲状腺原氨酸(FT_3)、游离甲状腺素(FT_4)和促甲状腺生成素(TSH),并对数据进行研究。结果 COPD组TT_3、FT_3的平均值低于对照组,并且与病情严重程度正相关,差异具有统计学意义(P<0.05);TT_4、FT_4和TSH平均值降低无统计学差异。随着COPD严重程度的增加,TT_3、FT_3平均值显著降低;COPD患者组中,TT_3、FT_3与肺功能指标、PaO_2及SO_2呈显著正相关,与PaCO_2呈显著负相关。结论 COPD患者甲状腺激素水平与COPD的严重性、进展和预后有一定关系。Objective To evaluate thyroid function in patients with COPD and to analyze the relationship between pulmonary function, arterial blood gas and thyroid hormone. Methods 70 patients with COPD ( grade 4) and 50 healthy volunteers (control group) were divided into two groups. FEV1, FVC, FEV1 / FVC, blood gas analysis parameters (PaCO2, PaO2, SO2 ), total triiodothyronine (TT3 ), total thyroxine (TT4 ), free triiodothyronine (PT3 ), free thyroxine (FT4 ) and thyrotropin (TSH) were measured, and the data was analyzed. Results The mean values of TT3 and PT3 in COPD group were lower than those in control group (P 〈 0.05 ), and was positively correlated with the severity of disease. And the mean values of TT4, FT4 and TSH were not significantly different. The mean values of TT3 and FT3 were significantly decreased with the increase of COPD severity. In COPD group, TT3, PT3, Lung function index, PaO2 and SO2 were significantly positively correlated, while PaCO2 was significantly negatively correlated. Conclusion Thyroid hormone levels in COPD patients are related to the severity, progression and prognosis of COPD.
