彩色多普勒超声诊断孤立性血尿患儿左肾静脉压迫综合征的临床价值

摘　　要：目的评价彩色多普勒超声诊断孤立性血尿患儿左肾静脉压迫综合征的临床价值。方法选择70例孤立性血尿患儿(患儿组)及30例健康儿童(正常对照组),仰卧位及脊柱后伸20分钟后,分别测量近肾门处左肾静脉扩张部位的内径(D_a)及狭窄处的内径(D_b),血流峰值流速(V_a、V_b),计算D_a/D_b、V_b/V_a。结果仰卧位:孤立性血尿患儿组D_a/D_b>2者占45.7%,正常组为6.7%;孤立性血尿患儿组V_b/V_a>3者占40.0%,正常组为3.3%。脊柱后伸20分钟后:孤立性血尿患儿组D_a/D_b>5者占38.6%,V_b/V_a>6者占35.7%,正常对照组各值无明显变化。仰卧位和脊柱后伸20分钟后,孤立性血尿患儿组各比值均明显高于正常对照组(P<0.05)。结论彩色多普勒超声是诊断孤立性血尿患儿是否患有左肾静脉压迫综合征的首要诊断依据。Objective To evaluate the clinical value of color Doppler ultrasound in the diagnosis of left renal vein entrapment syndrome in children with isolated hematuria. Methods 70 cases of children with isolated hematuria （isolated hematuria group） and 30 cases of healthy children （normal control group） were selected. On dorsal position and at 20 rain of posterior spinal extension, the diameter and peak velocity at expansion site （Da, Va） and stenosis site （Db, Vb） of left renal vein near to the renal hilum were measured, and Da/Db and Vb/Va were calculated. Results On dorsal position, the proportion of Da/ Db 〉 2 and proportion of ViVa 〉 3 were 45.7% and 40.0% of isolated hematuria group respectively, and 6.7% and 3.3% of normal control group respectively. At 20 rain of posterior spinal extension, the proportion of Da/Db 〉 5 and proportion of DJDb 〉 6 of isolated hematuria group were 38.6 % and 35.7% respectively, while the normal control group had no statistical change. The values of Da/Db and Da/Db on dorsal position and at 20 min of posterior spinal extension of isolated hematuria group were all significantly higher than those of normal control group （P 〈0.05）. Conclusions Color Doppler ultrasound is the preferred diagnostic basis for left renal vein entrapment syndrome in children with isolated hematuria.
