腹腔镜下TAPP治疗腹股沟疝的疗效观察与安全性评价

摘　　要：目的观察腹腔镜下TAPP(三维立体补片)治疗腹股沟疝患者的疗效与安全性。方法随机选取2014年10月至2016年8月我院收治的86例腹股沟疝患者,分为两组各43例。开放式组行开放式无张力疝修补术(充填式网塞)治疗,腹腔镜组行腹腔镜下TAPP治疗。比较两组患者的手术指标、并发症发生率及复发率。结果腹腔镜组的手术时间长于开放式组,住院时间短于开放式组,总住院费用高于开放式组,术中出血量低于开放式组,术后下床活动时间早于开放式组,差异均有统计学意义(P<0.05)。腹腔镜组的并发症发生率、复发率分别为7.0%、4.7%,低于开放式组的23.3%、20.9%,差异均有统计学意义(P<0.05)。结论与开放式手术比较,腹腔镜下TAPP治疗腹股沟疝患者具有较好的临床疗效,以及较低的并发症发生率与复发率。Objective To observe the efficacy and safety of laparoscopic TAPP （three-dimensional patch） in the treatment of inguinal hernia. Methods 86 cases of patients with inguinal hernia admitted to our hospital from October 2014 to August 2016 were randomly selected and divided into two groups. 43 cases of open group were treated with open tension-free hernia repair （filling mesh plug）, while 43 cases of laparoscopic group were treated with laparoscopic TAPP. The surgical indicators, incidence of complications, and recurrence rate were compared between two groups. Results The operation time oflaparoscopic group was significantly longer than that of open group, the length of hospital stay was significantly shorter than that of open group, the total hospitalization cost was significantly higher than that of open group, the intraoperative blood loss was lower than that of open group, and the postoperative ambulation time was significantly shorter than that of open group （all P 〈0.05）. The incidence of complications and recurrence rate of laparoscopic group were 7.0% and 4.7% respectively, significantly lower than 23.3% and 20.9% of open group, respectively （.P 〈0.05）. Conclusions Compared with open surgery, laparoscopic TAPP has better clinical curative effect, lower incidence of complications and lower recurrence rate in the treatment of ingainal hernia.
