超声诊断剖宫产术后子宫-腹壁粘连的探讨

摘　　要：目的探讨剖宫产术后子宫-腹壁粘连的超声图像特征。方法选择2014年7月至2016年7月期间我院收治的84例剖宫产术后子宫-腹壁粘连患者作为观察组,另选取80例同期来我院健康体检的有剖宫产史的产妇作为对照组,两组均行彩色多普勒超声检查,比较两组的检查结果。结果观察组患者中,超声检查图像特征包括:(1)当膀胱处于充盈状态时,子宫的位置显著高于正常人群的水平,子宫宫底的局部前壁和腹壁紧密贴合;(2)当患者深呼吸时,其宫体和腹壁之间的相对移动程度轻微,而憋气后宫底和宫体之间有明显的角度出现。观察组患者的宫颈长度与宫体纵径均明显大于对照组(P<0.05);而宫体横径和前后径与对照组比较无统计学差异(P>0.05)。结论剖宫产术后产妇的子宫和腹壁是否发生粘连可以通过彩色多普勒超声技术进行鉴别和诊断,此类患者的超声图像具有较高的特征性,诊断率高,可以作为首选的检查手段以帮助临床医师进行初筛。Objective To explore the ultrasound image features of uterine-abdominal wall adhesion after caesarian section. Methods 84 cases of patients with uterine-abdominal wall adhesion after caesarian section in our hospital from July 2014 to July 2016 were selected as observation group, and 80 cases of healthy controls with health examination after caesarian section at the same period were selected as control group. All cases received color Doppler ultrasound examination, and the results of examination were compared between two groups. Results Among 84 cases of observation group, the ultrasound image features were as follows： OWhen the bladder was filled, the position of the uterus was significantly higher than that of normal population, and the anterior wall of uterus and the abdominal wall were tightly attached. （~）When patients had deep breathing, the degree of relative movement between uterine and abdominal wall was slight, while there was a clear angle between uterine fundus and uterine body after holding breath. The cervical length and longitudinal diameter of uterine body of observation group were significantly longer than those of control group （P 〈0.05）, while the transverse diameter and anteroposterior diameter of uterine body showed no statistical difference between two groups （P〉0.05）. Conclusions Color Doppler ultrasound can identify and diagnose uterine-abdominal wall adhesion after caesarian section, with significant image features and high diagnostic rate, which can be used as the preferred inspection method for primary screening.
