归逆定眩汤对后循环缺血患者TCD影响

摘　　要：目的:观察归逆定眩汤对后循环缺血患者TCD主要检测指标的影响。方法:将98例符合气血亏虚型后循环缺血性眩晕的患者根据TCD检测结果,随机分为高流速治疗组、低流速治疗组和高流速对照组、低流速对照组,治疗组予归逆定眩汤治疗,对照组予氟桂利嗪胶囊口服,评估临床疗效,并通过TCD观察治疗前后双侧椎动脉及基底动脉的血流速度(Vs、Vm、Vd)、搏动指数(PI)、阻力指数(RI)的变化情况。结果:全部病例治疗后与治疗前比较血流速度均有显著改善(P<0.01),但治疗组与对照组间比较差异无统计学意义(P>0.05)。高流速治疗组PI、RI较治疗前均改善显著(P<0.01),对照组有部分改善,两组间比较部分差异具有统计学意义;低流速治疗组和对照组的PI、RI较治疗前均部分改善(P<0.01),两组间比较统计学差异不明显。结论:归逆定眩汤通过改善血流速度治疗气血亏虚型后循环缺血性眩晕疗效显著,可能对高流速患者的PI、RI改善更有效。Objective： To explore the effect to main outcome measurements of TCD on posterior circulation ischemia with Guini Dingxuan Decoction. Methods ： Based on the test results of TCD, 98 patients with posterior circulation ischemic vertigo with Qi and blood deficiency were randomly divided into high flow velocity treatment group, low flow velocity treatment group and high flow velocity control group, low flow velocity control group. The treatment groups received Guini Dingxuan Decoction treatment, and the control groups received Flunarizine capsules treatment. To evaluate the clinical curative effect. To observe the changes of blood flow velocities （ Vs, Vm, Vd ）, pulsatility index （ PI ） and resistive index （ RI ） of bilateral vertebral artery and basilar artery by TCD before and after treatment. Results ： After treatment, the blood flow velocities of all groups had statistically significant difference compared to before （ P〈0.01 ）, but it had no statistical significance between the treatment groups and the control groups （ P〉0.05 ）. The PI and RI of high flow velocity treatment group had statistically significant difference compared to before treatment （P〈0.01）, and it exhibited a partial improvement in high flow velocity control group, and some data had statistical significance between the two groups. In two low flow velocity groups, the PI and RI had a partial statistically significant difference compared to before treatment （ P〈0.01 ）, and it had no statistical significance between two groups. Conclusion： Guini Dingxuan Decoction is very effective for posterior circulation ischemic vertigo with Qi and blood deficiency by improving blood flow velocities. It perhaps does better in improving the PI and R1 of high flow velocity patients.
