针灸对颈椎管扩大成型手术患者术后康复影响

摘　　要：目的:为针灸治疗加速颈椎管扩大成形手术患者的术后康复提供临床依据。方法:采取分组对照的方式对2009—2012年在我科行脊髓型颈椎病后路椎管扩大成形术的患者进行临床疗效分析,病例总数为28例,随访时间均超过1年,采用JOA评分调查问卷的方式获得手术前后及末次随访的疗效反馈。结果:对采集的各项数据进行分析,其中男性患者多于女性,发病人群以中老年居多,病变范围广泛,通常超过3个脊髓节段,所有病例术后均行针灸治疗,治疗后均较术前有明显改善。对随访后的JOA数据进行分析,结果如下:术前评分(7.64±3.53)分,术后2周评分(12.04±4.33)分,改善率79.85%±8.4%;末次随访评分(9.98±6.03)分,改善率70.9%±6.3%。结论:针灸作为一种中医传统的治疗方法,能明显加快颈椎手术患者术后的神经功能恢复,使患者早期离床活动,达到快速康复的目的,同时避免了卧床合并症的发生,缩短围手术期时间,降低住院费用,进而使手术达到理想的治疗效果。Objective： To provide clinical evidence for postoperative rehabilitation on acupuncture treatment of patients with expansive cervical laminoplasty. Methods ： We divided the participants into two groups, and having clinical analysis on treatment of 28 patients with cervical spondylotic myelopathy treated by posterior laminoplasty from 2009 to 2012. All of these patients had a follow-up of more than 1 year. According to JOA questionnaire, we obtained curative feedback before and after operation also including last follow-up. Results： By analyzing various data, we found out that male cases were much more than female ones. The patients in the majority with middle-aged and old. The extent of disease usually more than three segments of the spinal cord. All of the patients were significantly improved after acupuncture treatment. According to the analysis of JOA data, the results were as follows ： The preoperative score was （ 7.64±3.53 ） points, after 2 weeks was （ 12.04±4.33 ） points, the improvement rates was （ 79.85%±8.4% ）; The last follow-up score was （ 9.98±6.03 ） points, the improvement rates was （ 70.9%±6.3% ）. Conclusion ： The treatment of acupuncture as a kind of traditional therapy, can obviously accelerate the recovery of nerve function after cervical operation. This method can help patients to achieve the purpose of rapid rehabilitation by promoting early arubulation. At the same time, it can achieve the desired therapeutic effect by avoiding the occurrence of bed complications, cutting the time of perioperative period and reducing the hospitalization costs.
