322例患者输注配合性血小板的临床研究

摘　　要：目的比较血小板抗体阳性和阴性患者输注配型相合血小板的临床治疗效果。方法以本院2014年1~12月收治的322例配合性血小板输注患者为研究对象,采用固相凝集法检测血小板抗体并进行血小板配型,对比血小板抗体阳性与阴性患者输注相合血小板的无效率,及血小板抗体阳性患者随机输注与配合性输注1 h和24 h后血小板增高指数(CCI)。结果 322例配合性血小板输注患者血小板抗体筛查阳性139例,阳性率为43.2%。血小板抗体阳性的患者输注配型相合的血小板,输注无效率为18.7%;血小板抗体阴性患者输注配型相合的血小板,输注无效率为3.83%;血小板抗体阳性患者输注无效率高于阴性患者,差异有统计学意义(P<0.01)。血小板抗体阳性患者配合性输注血小板的有效率明显高于随机输注(P<0.05),配合性输注有效率为81.3%。不同疾病间抗体阳性率差异无统计学意义(P>0.05)。结论采用固相凝集法检测血小板抗体并进行血小板配型,能够改善患者的输注效果,尤其是血小板抗体阳性患者,配合性输注血小板的有效率明显高于随机输注的效果。Objective To evaluate the clinical therapeutic effect of platelet antibody-positive or negative patients who had received transfusion of compatible platelet. Methods The total of 322 cases of the in-patients from Jan to Dec in 2014 were enrolled. Platelet antibodies were determined by solid-phase agglutination and corrected count increment （CCI value） after lh and 24h ofplatelet transfusion were compared, betweenrandom and compatible transfusions The rates of platelet transfusion refractoriness of either platelet antibody positive patients or negative patients, who had been transfused with compatible platelets, were analyzed. Results One hundred and thirty-nine cases （43.2%） of positive platelet antibodies were found in 322 cases of patients with platelet transfusion .The rate of platelet transfusion refractoriness was 18.7% and 3.83% respectively in antibody positive and negative groups. The frequency of positive antibodies was significantly higher than that of the negative patients （P〈0.01） . For platelet antibody positive patients, the efficiency of compatible platelet transfusion was significantly higher than that of random platelet transfusion （P〈0.05） .The efficiency was 81.3%. There was no significant difference in diseases （P〉0.05） . Conclusion Platelet antibodies were detected and platelet matching was processed by solid phase agglutination assay, which can significantly improve the transfusion effect of platelet antibody positive patients.
