益肾化瘀通络方对糖尿病肾病4期胱抑素C的作用

摘　　要：目的:观察益肾化瘀通络方对气阴两虚、瘀血阻络型糖尿病肾病4期患者的治疗效果评定及治疗前后患者血清胱抑素C(Cys C)的水平变化。方法:将入选的70例2型糖尿病肾病4期患者,按随机分组的原则分为对照组和治疗组,对照组给予西药常规治疗。治疗组在基础治疗的同时加用益肾化瘀通络方,治疗周期为8周。观察2组治疗前后中医症候变化、空腹血糖(FBS),糖化血红蛋白(Hb A1c)、血脂、胱抑素C(Cys C)的变化。结果:益肾化瘀通络方中药治疗组,在降低空腹血糖、血脂、糖化血红蛋白、胱抑素C(Cys C)水平方面较西药对照组疗效好,经统计学分析(P<0.05)有显著性差异。结论:益肾化瘀通络方除具有辅助降糖的作用外,还能降低患者的胱抑素C(Cys C)水平,改善临床症状。因此益肾化瘀通络方对气阴两虚、瘀血阻络型糖尿病肾脏病变有预防和治疗作用,在临床运用上值得推广。Objective： To observe the therapeutic effect of Yishen Huayu Tongluo decoction on the treatment of qi and yin deficiency and blood stasis type and diabetic nephropathy and the changes of serum Cystatin C （Cys-C） be- fore and after treatment. Methods： 70 patients with type 2 diabetic nephropathy were randomly divided into control group and treatment group. The control group was treated with conventional western medicine. The treatment group was treated with Yishen Huayu Tongluo decoction while the treatment period was 8 weeks. The changes of TCM symptoms, fasting blood glucose （FBS）, glycosylated hemoglobin （HbAlc）, blood lipid and Cys-C were observed before and after treatment. Results： The therapeutic effect of Yishen Huayu Tongluo group was better than that of the western medicine control group in reducing the fasting blood glucose, blood lipid, glycosylated hemoglobin and Cys-C （P〈0.05） There was a significant difference. Conclusion： Yishen Huayu Tongluo decoction can reduce the level of Cys-C and improve the clinical symptoms in addition to the effect of hypoglycemia. Therefore, Yishen Huayu Tongluo decoction on qi and yin deficiency, blood stasis type of diabetic nephropathy have a preventive and therapeutic effect, in the clinical appli- cation worthy of promotion.
