摘  要:目的分析人类免疫缺陷病毒(HIV-1)急性期感染者(acute HIV-1-infected patients,AHIs)肠道归巢α4β7 Th17细胞的变化特点。方法本研究共纳入26例HIV-1急性期感染者、20例HIV-1慢性期感染者和20例健康对照者。采用流式细胞仪检测肠道归巢α4β7Th17细胞的比例和数量,并分析肠道归巢α4β7Th17细胞与CD4^+T细胞、T细胞免疫活化水平之间的相关性。结果与健康对照者比较,HIV-1急性期感染者肠道归巢α4β7Th17细胞的比例(2.61%vs 1.34%,P<0.000 1)和数量(2.29 vs 0.66,P<0.000 1)均显著减少,肠道归巢α4β7 Th17细胞与肠道归巢α4β7 CD4^+T细胞(r=0.42,P=0.02)和CD4^+T细胞(r=0.60,P=0.002)呈显著正相关。HIV-1急性期感染导致T细胞免疫活化水平升高(1.87%vs 15.60%,P<0.000 1),肠道归巢α4β7 Th17细胞与T细胞免疫活化水平呈负相关(r=-0.47,P=0.01)。结论肠道归巢α4β7 Th17细胞的丢失可能在HIV-1致病机制中起作用。Objective To analyze the change of gut-homing α4β7 Th17 cells in individuals with acute HIV-1 in- fection. Methods A total of 26 acute HIV-l-infeeted patients, 20 chronic HIV-1 infected patients, and 20 healthy controls were enrolled in this study. Flow cytometry was performed to analyze the frequency and abso- lute number of gut-homing α4β7 Thl7 cells. Correlations between gut-homing α4β7 Th17 cells and CD4+ T cell count or the level of T cell activation were also analyzed. Results Comparing to healthy controls, gut-hom- ing α4β7 Th17 cells were significantly decreased in acute HIV-1-infected patients in both frequency and abso- lute number (2.61% vs 1.34%, P〈0. 000 1). Gut-homing Th17 cells in acutely infected patients were posi- tively correlated with gut-homing CD4+ T cells (r=0.42, P=0.02) or circulating CD4+ T ceils (r=0.60, P=0. 002). HIV-1 acute in{ection also led to elevated level of T cell immune activation (P〈0. 000 1). Gut-homing Thl7 cells during acute infection stage had a negative correlation with the level of T cell immune activation (r=-0.47, P=0.01). Conclusions Gut-homing α4β7 Th17 cells may contribute to the HIV-1 pathogenesis.
