摘 要:目的了解轻、重度病毒性脑炎患儿脑脊液和血清中神经元特异性烯醇化酶(NSE)、脑型肌酸激酶同功酶(CKBB)、乳酸脱氢酶(LDH)同工酶的变化。方法回顾性分析2010年1月至2015年12月沧州市中心医院收治的30例病毒性脑炎重症患儿、30例轻症患儿和30例排除颅内感染的偏头痛患儿,并对其脑脊液和血清中NSE、CK-BB、LDH同工酶进行检测,采用多因素方差分析进行多组间均数的分析及比较。结果在血清分析中,三组患儿NSE、CK-BB、LDH同工酶检测结果有统计学差异(F值分别为168.62、73.82和27.08,均P<0.01);在脑脊液分析中,三组患儿NSE、CK-BB、LDH同工酶检测结果也有统计学差异(F值分别为291.72、891.83和60.544,均P<0.01)。结论病毒性脑炎患儿脑脊液和血清中NSE、CK-BB、LDH变化可作为诊断病毒性脑炎及评估病情轻重的临床指标,可广泛应用于临床。Objective To analyze the changes of neuron specific enolase (NSE), creatine kinaes BB isozyme (CK-BB) and lactate dehydrogenase (LDH) isozyme in cerebrospinal fluid and serum of children with mild or severe viral encephalitis.Methods Retrospective analysis was conducted in 30 cases of severe viral encephalitis, 30 mild cases and 30 cases of migraine without intracranial infection admitted in Cangzhou Central Hospital from January 2010 to December 2015.Isozyme of NSE, CK-BB and LDH in cerebrospinal fluid and serum of patients was detected and means were analyzed and compared among groups using multiple factor variance analysis.Results Differences in serum NSE, CK-BB and LDH isozyme test results were statistically significant among three groups (F value was 168.62, 73.82 and 27.08, respectively, all P〈0.01).Differences in cerebrospinal fluid test results of isozyme of NSE, CK-BB and LDH had statistical significance (F value was 291.72, 891.83 and 60.544, respectively, all P〈0.01).Conclusion Changes of NSE, CK-BB and LDH in cerebrospinal fluid and serum of children with viral encephalitis could be used as clinical indexes in diagnosing viral encephalitis and evaluating severity of illness, and can be widely used in clinical application.
