摘 要:目的对比观察视可尼视频喉镜和Block Bust喉罩在阻塞性睡眠呼吸暂停低通气综合征患者气管插管中的临床应用。方法拟全麻下行腭咽成形术的阻塞性睡眠呼吸暂停低通气综合征患者60例随机分为视可尼视频喉镜组(S组,n=30)和Block Bust喉罩组(B组,n=30),两组患者均采用七氟醚吸入麻醉诱导,S组置入视可尼视频喉镜行正中入路气管插管;B组置入Block Bust喉罩,并经纤维支气管镜引导下行气管插管。对比观察两组患者的一次插管成功率、总插管成功率、插管成功时间和低氧血症发生率。结果两组一次插管成功率和总插管成功率比较,无显著差异(P>0.05);S组的插管时间(33.10±7.58 s)明显长于B组(24.03±3.54 s),差异有统计学意义(P<0.05);S组低氧血症发生率(20%)高于B组(3.33%),差异有统计学意义(P<0.05)。结论 Block Bust喉罩及视可尼视频喉镜均可用于阻塞性睡眠呼吸暂停低通气综合征患者气管插管,Block Bust喉罩插管时间更短,低氧血症的发生率更低。Objective To compare the significance of clinical application of Shikani and Block Bust laryngeal mask (LMA) in intubation for patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods 60 patients with OSAHS undergoing uvulopalato - pharyngoplasty under general anesthesia were randomly divided into Shikani group (S, n = 30) and Block Bust LMA group (B, n = 30). Patients in these two groups were applied with sevoflurane inhalation for anesthesia induction. The tracheal intubation in patients of group S was using Shikani with the middle road placement. Patients in group B were implanted with Block Bust laryngeal mask, then tracheal intubation was guided by fiberoptic bron-choscopy. The duration for successful attempt, the successful rate for intubation in the first attempt, the overall successful rate for intubation and the incidence of hypoxemia were recorded and compared between these two groups. Results The rate of first successful tracheal intubation and the over-all successful rate for intubation in these two groups had no significant difference ( P〉 0.05). The duration of successful tracheal intubation was 33.10 ±7.58 s (group S) and 24.03 ± 3.54 s (group B) respectively, there was significant difference between these two groups ( P 〈0.05 ) .The incidence of hypoxemia in group S (20% ) was significantly higher than that of group B (3.33% ) ( P〉0.05) . Conclusion Both Shikani and Block Bust LMA are safe and effective means for intubation in patients with OSAHS undergoing uvulopalato - pharyngoplasty. The duration for successful tracheal intubation and the incidence of hypoxemia in patients with Block Bust LMA were shorter and lower in comparison with those applied Shikani.
