膝关节腔注射联合侧副韧带痛点阻滞治疗老年性膝骨关节病对关节功能恢复的影响

摘　　要：目的观察应用膝关节腔注射联合侧副韧带痛点阻滞治疗老年性膝骨关节病对关节功能恢复的影响。方法选择膝骨关节病患者45例,随机分为两组:单纯膝关节腔注射组(A组,n=22),膝关节腔注射联合侧副韧带痛点阻滞组(B组,n=23),侧副韧带阻滞可根据患者的痛点位置定位选择。对比观察:(1)两组治疗前及治疗后1、2、3、4周膝关节静态和动态时VAS疼痛评分;(2)两组治疗前及治疗后膝关节功能恢复程度评分。结果 (1)组内比较:A组治疗后4周与治疗前比较静止痛和运动痛VAS疼痛评分值有显著减低,膝关节功能活动评分明显提高(P<0.05);B组治疗后1、2、3、4周治疗前比较静止痛和运动痛VAS疼痛评分值有显著减低,膝关节功能活动评分明显提高(P<0.05);(2)组间比较:B组治疗后1、2、3、4周与A组比较,静止痛和运动痛VAS疼痛评分值均有显著减低,膝关节功能活动评分明显提高(P<0.05)。结论应用膝关节腔注射联合侧副韧带痛点阻滞方法治疗老年性膝骨关节病可以多方位控制膝关节及其周围的炎性疼痛症状,关节功能恢复得更为有效。Objective To observe the therapeutic effect of intra - articular injection combined with trigger point block in collateral ligaments for recovery of the function of knee joint in aged patients with osteoarthritis. Methods Forty - five outpatients with pain from aged knee joint osteoarthritis were randomly assigned into group A （n = 22） and group B （n = 23） , patients in group A were performed with intra - articular injection, and patients in group B were applied with the combination of intra - articular injection and trigger point block in collateral ligaments. The visual analogue scale （ VAS） scores of patients at rest （ VAS - R） and in activity （ VAS - A） before treatment, two weeks after treatment, and four weeks after treatment were assessed and compared between these 2 groups. Secondly, the functional recovery scale of knee joint （FRS - KJ） was assessed at the time before treatment, two weeks and four weeks after treatment. Results Patients in group A had lower VAS - R, lower VAS -A, and higher FRS - KJ in four weeks after the treatment than those before treatment （ P 〈 0. 05） , while patients in group B had lower VAS - R, lower VAS - A, and higher FRS - KJ scores in two weeks and four weeks after treatment than those before treatment （ P 〈 0 .0 5 ） . VAS - R and VAS - A scores in two weeks and four weeks after treatment in patients of group B were lower than those of patients in group A （ P 〈0. 05） and FRS - KJ in two weeks and four weeks after treatment in patients of group B were higher than those of patients in group A （ P 〈 0. 0 5 ） . Conclusion The application of intra - articular injection combined with trigger point block in collateral ligaments can decrease the inflammatory pain around knee joint from various aspects, and the recovery of function of knee joint is more effective.
