生脉保元汤加味对扩张型心肌病合并心力衰竭患者心功能及血清BNP、CA125的影响

摘　　要：目的观察生脉保元汤加味对扩张型心肌病合并心衰患者心功能及血清B型脑钠肽(BNP)、糖链抗原125(CA125)的影响。方法将62例扩张型心肌病合并心衰患者随机分为观察组和对照组各31例,2组给予常规治疗,观察组在此基础上加用生脉保元汤加味治疗,连续治疗8周。观察2组治疗前后心功能指标、心率变异性指标及血清BNP、CA125变化情况,并评估临床疗效。结果观察组治疗后总有效率显著高于对照组(P<0.05);2组治疗后LVEDD和LVESD均显著降低(P均<0.05),LVEF和FS均显著升高(P均<0.05),且观察组的改善情况明显优于对照组(P均<0.05);2组治疗后SDNN、r MSSD及PNN50均显著升高(P均<0.05),血清BNP和CA125水平均显著降低(P均<0.05),且观察组改善情况明显优于对照组(P均<0.05)。结论生脉保元汤加味治疗扩张型心肌病合并心衰疗效确切,能够调节患者神经内分泌功能,改善患者心功能。Objective It is to observe the effect of Shengmai Baoyuan decoction on cardiac function and serum B - type brain natriuretic peptide （BNP）, carbohydrate antigen 125 （CA125） in patients with dilated cardiomyopathy （DCM） with heart failure. Methods 62 patients with DCM with heart failure were randomly divided into observation group and control group, each group had 31 cases. Both groups were given routine therapy, additionally the observation group was treated with Shengmai Baoyuan decoction on the basis of treatment in control group. Both groups were treated for 8 weeks. The changes of indicators of heart function, heart rate variability and serum BNP, CA125 were observed, and clinical efficacy was evaluated in both groups after treatment. Results The total effective rate was significantly higher in observation group than that in control group （P 〈 0.05）. LVEDD and LVESD were significantly decreased while LVEF and FS were increased in both groups after treatment （ P 〈 0.05 ） , and the improvements of observation group were obviously better than that of control group （ P 〈 0.05）. SDNN, rMSSD and PNN50 were significantly increased while the serum levels of BNP and CA125 were decreased in both groups after treatment （P 〈 0.05）, and the improvements of observation group were obviously better than that of control group （P 〈 0.05 ）. Conclusion Shengmai Baoyuan decoction could improve neuroendocrine function and heart function, and has good curative effect on DCM with heart failure.
