摘　　要：目的探讨鞍上区实性肿瘤CT和MRI特征。方法回顾性分析35例鞍上区实性肿瘤CT和MRI表现,其中12例脑膜瘤,2例颗粒细胞瘤,1例中枢神经细胞瘤,1例脊索样胶质瘤,2例颅咽管瘤,5例生殖细胞瘤,7例视神经胶质瘤,2例淋巴瘤,3例转移瘤。结果脑膜瘤CT呈等密度影,MRI呈等T1、等T2信号,均匀明显强化,可有"脑膜尾征";颗粒细胞瘤与鞍隔及垂体柄解剖关系密切,SWI见低信号出血坏死灶;脊索样胶质瘤呈持续性明显强化,可有钙化;中枢神经细胞瘤,MRI呈等T1、等T2信号,增强后明显强化;颅咽管瘤呈等T1、稍长T2信号,明显强化;生殖细胞瘤增强扫描明显强化,SWI见多发静脉血管及微出血灶;视神经胶质瘤沿视神经生长;淋巴瘤及转移瘤多呈结节状强化。结论鞍上区实性肿瘤CT和MRI表现具有一定特征性。Objective Explore CT and MRI features for solid tumor in suprasellar region. Methods a retrospective analysis of 35 cases of CT and MRI performance of solid tumor in the suprasellar region, including 12 cases of meningioma, 2 cases of granular cell tumor, 1 case of central nervous cell tumor, 1 case of ehordoid glioma, 2 cases of craniopharyngioma, 5 eases of germ cell tumors, 7 cases of optic glioma, 2 eases of lymphoma, 3 cases of metastatic tumor. Results Meningioma CT showed intermediate density masses, isointense TIWI and T2WI signals, obvious homogeneous enhance- ment, and may have ＂dural tail sign＂. Granular cell tumor is closely related to the anatomy of the diaphragma sella and the pituitary stalk, from SWI we observed a low signal focal hemorrhage and necrosis. Granular cell tumor after enhancement showed ＂ fast in fast out＂ enhancement feature. Chordoid glioma had the persistent avid enhancement, can have calcification; neural cell tumor, isointense signal on T1WI ＆ T2WI. Craniopharyngioma showed an isointense signal on T1 WI and slightly long T2WI signal with strong enhancement. Germ cell tumor post-contrast studies showed obvious enhancement, SWI showed multiple microbleeds. Optic nerve glioma grew along the optic nerve. Lymphoma and metastatic tumor showed nodular enhancement. Conclusion CT and MRI studies for solid tumors in the suprasellar region have particular characteristic features.
