血必净注射液治疗急性百草枯中毒的临床研究

摘　　要：目的:探讨血必净注射液在急性百草枯中毒治疗中的疗效。方法:62例百草枯(PQ)中毒患者随机分为对照组、实验组,对照组给予洗胃、血液灌流、激素等治疗,实验组在以上基础治疗上加用血必净,治疗过程中监测患者各项化验指标、SOFA评分,比较不同时期各化验指标、肺损伤/纤维化、SOFA评分及死亡率。结果:在治疗的第3天、第7天,实验组肌酐(Cr)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、氧分压与对照组相比明显好转,差异有统计学意义;在第7天、第14天,实验组与对照组SOFA评分相比,P=0.039,P=0.042,实验组与对照组死亡率比较,P=0.036,P=0.025,差异有统计学意义,第28天两组死亡率比较,差异有统计学意义(P=0.043)。结论:血必净在中毒初始大剂量应用,可减轻炎症介质释放,防止肺损伤/纤维化,保护各脏器功能,降低早期死亡率。Objective：To investigate the clinical effect of acute paraguat poisoning treatment with xuebijing injection. Methods：All 62 patients with acute paragual poisoning were divided into the control group and the experimental group. The control group was given gastric lavage, hemoperfusion, hormone treatmenet ; the experimental group was given xuebijing injection on the basis of above treatment. Monitoring the laboratory indicators, SOFA scores, comparing of different period＇s laboratory in- dicators,lung injury/fibrosis, SOFA scores and mortality rate. Results：On the third, seventh day, Cr, CK, CK - MB and PaO2 of experimental group were better than the control group, have statistical difference. On the seventh, fourteenth day,there were sta- tistical difference between two groups＇ SOFA scores （ P = 0. 039,0.042）, mortality rate （ P = 0. 036,0.025 ）. On the twenty - eighth day, there were statistical difference （P = 0.043 ） in mortality rate between two groups. Conclusion：Using Of a large dose xuebijing onset of initial, can ruduce the release of inflammatory mediators, prevent lung injury / fibrosis, protect the function of each organ, ruduce the early mortality rate.
