体外冲击波碎石术后肾肝宁胶囊对肾脏保护作用的研究

摘　　要：目的探讨肾肝宁胶囊辅助体外冲击波碎石(ESWL)治疗肾结石的疗效,以及对肾脏的保护作用。方法选取180例符合纳入标准的肾结石患者作为研究对象,按照随机数字表法分为对照组和观察组,每组90例。常规治疗基础上,对照组给予ESWL治疗,观察组在对照组基础上给予肾肝宁胶囊辅助治疗。比较对照组和观察组患者临床疗效、尿N-乙酰-β-葡萄糖苷酶(NAG)、尿β2-微球蛋白(β2-MG)、炎症介质及氧化应激指标水平。结果 12 d后疗效评价显示,观察组和对照组的治疗总有效率分别为96.7%和94.4%,差异无统计学意义(P>0.05)。治疗7 d后,与对照组比较,观察组患者尿NAG、尿β2-MG水平降低,炎症介质肿瘤坏死因子-α(TNF-α)、一氧化氮NO水平降低(P<0.05)。观察组患者治疗后MDA水平低于对照组,而SOD水平高于对照组(P<0.05)。结论肾肝宁胶囊辅助ESWL治疗肾结石的有效率高,并可能通过减轻机体炎症反应及氧化应激,减少ESWL对肾脏造成的损伤,值得进一步研究。Objective To investigate the clinical effect and kidney protection effect of adjuvant therapy of shenganning capsules for renal calculi treated with extracorporeal shock wave lithotripsy （ESWL）. Methods 180 patients with renal calculi who met the inclusion criteria were collected as research objects. According to random number table, patients were randomly divided into two groups, equally with 90 cases in each group. Besides conventional treatments, the control group was treated with ESWL, while the observation group was treated with shenganning capsules combined with ESWL. The curative efficacy, levels of urine N-acetyl-β- glucosaminidase （NAG）, urine β2-microglobulin （β2-MG）, inflammatory mediators and oxidative stress indices were compared. Results After the appraisal, the therapeutic efficiency ratio in the observation group and the control were 96.7% and 94.4% respectively with no statistical difference （P〉 0.05）. Compared with the control group, the observation group had statistically lower levels of urine NAG, urine β2-MG （P〈 0.05） and had statistically lower levels of inflammatory mediators of TNF-α, NO （P〈 0.05） after the treatments. In comparison with the control group after the treatments, the observation group had statistically lower level of MDA （P 〈 0.05） and statistically higher level of SOD （P 〈 0.05）. Conclusion Adjuvant therapy of shenganning capsules for renal calculi treated with extracorporeal shock wave lithotripsy is effective, which has a high therapeutic efficiency ratio. And it may reduce kidney injury through alleviating inflammatory reaction and oxidative stress.
