超声评价2型糖尿病患者合并非酒精性脂肪肝及与心脏瓣膜钙化的相关性研究

摘　　要：目的探讨2型糖尿病(T2DM)患者合并非酒精性脂肪肝(NAFLD)及与心脏瓣膜钙化(HVC)的相关性。方法 150例T2DM患者,依据病程分为两组,病程>10年(A组)75例,病程≤10年(B组)75例,同时选取75例健康志愿者作为对照组(C组)。对3组患者行肝脏超声及经胸超声心动图检查,根据二维图像特征将NAFLD分为轻度、中度及重度,获取HVC评分,并进行相关性分析。结果 A、B两组间NAFLD和HVC的阳性率及严重程度均高于C组,且A组高于B组,差异有统计学意义(P<0.05)。病程与NAFLD及HVC均呈正相关关系(r=0.449和0.338,P<0.05),且NAFLD和HVC存在正相关关系(r=0.233,P<0.05)。结论随着T2DM患者的病程增加,NAFLD和HVC的程度加重,超声技术可以客观地评价T2DM患者合并的这两种异常改变,具有重要的临床意义。Objective To investigate the correlation between nonalcoholic fatty liver disease （NAFLD） and heart valvular calcification （HVC） in patients with type 2 diabetes mellitus （T2DM）. Methods 150 patients with T2DM were enrolled and divided into two groups according to the course of the disease： group A （75 cases, the course 〉 10 years） and group B （75 cases, the course ≤10 years）. And 75 healthy volunteers were enrolled as the control group （group C）. Ultrasonography and transthoracic eehocardiography were performed in all three groups. Non-alcoholic fatty liver was divided into mild, moderate and severe according to the 2-dimensional uhrasonography. HVC scores were obtained at the same time and the correlation was analyzed. Results The morbidity and severity of NAFLD and HVC of group A and B were significantly higher than that in group C, and group A was higher than group B （P〈 0.05）. And there were positive correlations between the course and NAFLD and HVC respectively （ra = 0.449 and 0.338, P〈 0.05）. There was a positive correlation between NAFLD and HVC （rs = 0.233, P〈 0.05）. Conclusions As the course of T2DM increases, the degree of NAFLD and HVC increases. And uhrasonography can objectively evaluate these two abnormalities in patients of T2DM with clinical significance.
