102例癌因性疲乏的中医证候规律研究

摘　　要：目的:探讨癌因性疲乏的中医辨证要素及证候分布规律。方法:对102例癌因性疲乏患者的中医基本辨证要素及证候分布规律进行回顾性研究。结果:中医基本辨证要素中病性证素分布呈气虚>痰浊>瘀血>阴虚>气滞>阳虚>血虚>热毒的趋势;病位证素分布呈脾>胃>肾>肝>肺>肠>心的趋势。证型分布中虚证呈现气阴两虚证>气血不足证>肝肾阴虚证>脾肾阳虚证的趋势;虚实夹杂证呈脾虚痰湿凝聚证>肝郁脾虚证>气虚血瘀证的趋势;实证明显少于前两者,在研究的病例中仅有痰热蕴结证。结论:癌因性疲乏中医证候规律以虚证最多,尤以气虚为主,且同时兼夹痰湿、瘀血等病理因素。故治疗癌因性疲乏以补虚为基本治则,既要重视脾胃运化,亦要兼顾化痰祛湿,活血化瘀,疏肝解郁等治则。Objective： To investigate the characteristic of cancer-related fatigue symptoms （CRF） and the distribution of Traditional Chinese Medicine symptoms. Methods： We retrospectively reviewed the syndrome differ- entiation of traditional Chinese medicine on 102 cases of CRF. Results： The distribution of nature element of pattern had a decrease trend of deficiency of Qi, phlegm evil, blood stasis, deficiency yin, stagnation of the cir- culation of vital energy, deficiency Yang, deficiency of blood and heat toxin. Also, the distribution of disease lo- cation had a decrease trend of spleen, stomach, kidney, liver, lung, intestines and heart. In terms of the distri- bution of deficiency syndrome deficiency of both Qi and Yin, deficiency of Qi and blood, deficiency of liver-Yin and kidney-Yin, deficiency of spleen-Yang and kidney-Yang decrease in sequence. The distribution of syndrome of intermingled deficiency and excess is insufficiency of the spleen and phlegmatic hygrosis 〉stagnation of liver qi and spleen deficiency〉Qi deficiency and blood stasis. Sthenia syndromes are significantly less than the former two, there is only phlegm heat accumulate knot syndrome in the study. Conclusion： Deficiency syndrome is the most in Traditional Chinese Medicine symptoms of CRF, especially deficiency of Qi which coexists phlegm evil and blood stasis. Therefore, the treatment principle of CRF is invigorating Qi , simultaneously, more attention should be paid to spleen and stomach, promoting blood circulation to remove blood stasis, soothing liver-Qi stagnation.
