摘 要:目的探讨改良DTM培养基对皮肤癣感染的诊断价值。方法采用三点接种法,将临床常见的8种皮肤癣菌、9种非皮肤癣菌和11份临床皮肤病皮屑标本接种于DTM培养基及2种改良DTM培养基上,28℃培养7d,每24h观察一次菌落生长及三种培养基颜色变化情况。结果皮肤癣菌中除紫色毛癣菌外均可使原DTM(橙黄→红色)和溴百里酚蓝改良DTM变色(土黄→深蓝),而紫色石蕊改良后DTM无颜色变化。其中须癣毛癣菌、许兰毛癣菌与絮状表皮癣菌在溴百里酚蓝改良DTM上培养3d变色明显,而在原DTM上需培养4d才会出现明显颜色变化;非皮肤癣菌致DTM变色均需培养5d以上。结论皮肤癣菌使溴百里酚蓝改良DTM变色跨度大,肉眼易判读;且比原DTM培养基更为敏感,有利于皮肤癣菌感染的快速诊断,值得大力推广应用。Objective To study the traditional dermatophyte test medium(DTM) and its two modified(DTM) medium in identifying common dermatophytes. Methods Totally 8 dermatophytes species,9 non-dermatophytes fungi and 11 clinical samples were inoculated on DTM and the two modified DTM media,and all tests repeated three times. Results The medium color changed in both DTM(yellow to red) and DTM-BB(gray yellow to mazarine blue) with dermatophyte growth except Trichophyton violaceum,while no change occurred in DTM-L. The turning points of Trichophyton mentagrophytes,Trichophyton schoenleinii and Epidermophyton floccosum were 3 days in DTM-BB,4 days in DTM;However,the turning points of non-dermatophyte were longer than 5 days. The same results obtained when clinical samples were tried. Conclusion The DTM modified with bromothymol blue(DTMBB) was more specific and sensitive to identify dermatophytes species than the traditional DTM medium,and it could be applied in clinics as a dermatophyte test technique which is helpful for distinguishing dermatophytosis from non-dermatophytes infections.
