体外冲击波治疗桡骨茎突狭窄性腱鞘炎的临床疗效

摘　　要：目的：观察体外冲击波（ESWT）治疗桡骨茎突部狭窄性腱鞘炎的临床效果。方法：将2014-01-2015-12收集的80例病人随机分为治疗组和对照组（n=40）,治疗组给予ESWT治疗,对照组给予局部外用吲哚美辛软膏。比较两组病人治疗前与治疗后第2wk、第4wk的视觉模拟评分（VAS）和腕关节功能评分（Cooney）,并评价两组病人治疗后第4wk的临床疗效。结果：两组病人治疗后第2wk、第4wk的VAS评分和Cooney腕关节评分均较治疗前明显改善（P〈0.05）,且治疗组的VAS评分和Cooney腕关节评分较同期对照组病人改善程度明显（P〈0.05）;治疗组病人治疗后第4wk的VAS评分和Cooney腕关节评分均较第2wk改善,差异有统计学意义（P〈0.05）,但对照组病人的VAS评分和Cooney腕关节评分无明显差异（P〉0.05）。在治疗后第4wk,虽然两组病人的有效率相比无明显差别（95%vs90%）（P〉0.05）,但治疗组病人的痊愈率明显高于对照组（77.5%vs 67.5%）（P〈0.05）,且在治疗6mo后回访,观察组复发率明显低于对照组（P〈0.05）。结论：与局部外用非甾体类药物相比,ESWT治疗桡骨茎突狭窄性腱鞘炎疗效更佳,且复发率低,值得临床推广应用。Objective：To observe the effect of extracorporeal shock wave（ ESWT） clinical effect of the treatment of stenosing tenosynovitis of the styloid process of radius. Methods：80cases of patients collected from 2014-01-2015-12 were randomly divided into treatment group and control group（ n=40 ） ,the treatment group received ESWT treatment,while the control group was treated with topical in-dometacine ointment. Compared the two groups before and after treatment with the treatment of the first 2wk,the visual analogue scale（4wk）and wrist function score（Cooney）,and to evaluate the clinical efficacy of the two groups of patients after the treatment of 4wk. Results：The treatment group two patients after 2wk,VAS score and Cooney wrist score in 4wk were significantly improved after treatment （P〈0. 05）,VAS score and Cooney wrist score and the treatment group compared with the control group of patients improved significantly（P〈0. 05）. Patients in the treatment group after treatment 4wk score of VAS and Cooney were lower than the 2wk wrist score improved. The difference was statistically significant（P〈0. 05）,but no significant difference between the VAS score and Cooney wrist score in control group patients（P〉0. 05）. After treatment the 4wk,the effective rate of the two groups were compared no significant difference（95% vs 90%）（P〉0. 05）,but the patients in the treatment group cure rate was significantly higher than control group（77. 5%vs 67. 5%）（P〈0. 05）,and follow-up in 6mo after treatment,the recurrence rate of the observation group was significantly lower than that of control group（P〈0. 05）. Conclusion：Compared with topical non steroidal drugs,ESWT treatment is better curative effect,low recurrence rate. worthy of clinical application,so it is worthy to be extended.
