摘 要:目的研究辛伐他汀对骨质疏松兔下颌骨骨折愈合过程中骨痂骨密度及骨保护素(OPG)表达情况的影响。方法采用60只6月龄雌性新西兰大白兔,体重在2.5kg左右,实验动物按照随机原则分为三组:假手术+颌骨骨折组(S+MF)、去势+颌骨骨折组(OVX+MF)、去势+颌骨骨折+辛伐他汀组(OVX+MF+SVS),每组20只。其中OVX+MF+SVS组和OVX+MF组建立骨质疏松模型,骨质疏松模型建立成功后再将三组动物分别做下颌骨骨折模型。术后第一天开始去势+颌骨骨折+辛伐他汀组给予辛伐他汀10mg/(kg·d)灌胃;另外二组给予等量的0.9%氯化钠灌胃。实验动物分别在术后2、4、6、8周每组各处死5只。取出左侧下颌骨后去净软组织后立即投入4%多聚甲醛中,先行骨密度检查,然后将标本投入4%的多聚甲醛液中固定48h。10%乙二胺四乙酸(EDTA)标本脱钙60d,脱钙完成后组织块石蜡包埋,行OPG免疫组织化学检测。结果骨折区骨密度检测:测量2、4、6、8周三组骨折区骨密度:术后6周三组骨密度分别为OVX+MF组:0.21 078±0.00 671;OVX+MF+SVS组:0.28 532±0.00 166;S+MF组:0.28 584±0.00 168;术8周三组骨密度分别为OVX+MF组:0.24 696±0.00 371;OVX+MF+SVS组:0.31 552±0.00 477;S+MF组:0.31 968±0.00 538;其中S+MF组和OVX+MF+SVS组骨密度明显高于OVX+MF组,有统计学意义;S+MF组和OVX+MF+SVS组之间差异无统计学意义。免疫组织化学检测:骨保护素(OPG)在骨痂区2、4、6周的表达:OVX+MF+SVS组和S+MF组均高于OVX+MF组,差异有统计学意义。结论辛伐他汀可以增加兔骨质疏松性下颌骨骨折区OPG的表达,有利于兔骨质疏松性下颌骨骨折愈合。Objective To Investigate the effect of simvastatin on BMD,OPG in osteoporosis mandible fracture healing rabbits. Methods Sixty 6 - monthold female New Zealand rabbits were randomly divided into three groups,Sham - operated mandible fracture group (S + MF ), Ovariectomized and mandible fracture group ( OVX + MF), Ovariectomized, mandible fracture and Simvastatin therapy group ( OVX + MF + SVS). OVX + MF group and OVX + MF + SVS group established model of osteoporosis. And the model of osteoporosis was established successfully. All the rabbits were made mandible fracture at the mandible. A day later,simvastatin was administered orally 10mg/kg/d to OVX + MF + SVS group and saline to the other groups. At 2,4,6,8 weeks,the rabbits" tissue was detected by the immunohistochemics for OPG. Results The bone mineral density of 3 groups at 2,4,6,8 weeks showed that the BMD of OVX+ MF group was 0.21078 ± 0.00671 at 6 weeks post operation,0.31552 ± 0.00477 in OVX + MF + SVS group,0.28584 ± 0.00168 in S + MF group;8 weeks post operation the BMD of OVX + MF group was 0.24696 ± 0.00371,0.31552± 0.00477 in OVX + MF + SVS group,0.31968 + 0.00538 in S + MF group. The BMD of N + MF group and OVX + MF + SVS group were higher than OVX + MF group (P 〈0.05), but no significance between S + MF group and OVX + MF +SVS group (P〉0.05). Immunohistochemical assessment:the express of OPG at 2,4 and 6 weeks in the S + MF group and OVX+ MF+ SVS group were higher than OVX+ MF group (P〈0.05). Conclusion Simvastatin can increase the express of OPG at fracture area, promote the formation of bone. Simvastatin can confront the blight of osteoporosis fracture healing.
