关节镜手术治疗踝关节前方撞击综合征的疗效观察

摘　　要：目的探讨关节镜手术治疗踝关节前方撞击综合征的临床疗效。方法回顾性分析我院2011年7月~2015年9月28例关节镜手术治疗踝关节前方撞击综合征的临床资料,男19例,女9例。年龄18~49岁,平均31岁。软组织撞击18例,骨性撞击10例。采用美国足踝外科(AOFAS)踝-后足评分标准和Meislin疗效评估标准评定疗效。结果 28例术后均获得随访,随访时间9~28个月,平均18个月。术前AOFAS踝-后足评分38~65分,(49.2±6.9)分,末次随访为72~95分,(88.5±5.5)分,明显提高(t=-26.000,P=0.000)。采用Meislin疗效评估,优20例,良5例,可2例,差1例,优良率89.3%(25/28)。均未出现神经损伤及切口感染。结论关节镜是治疗踝关节前方撞击综合征的有效措施,操作简便,疗效可靠,术后功能恢复快,满意率高,值得在临床推广。Objective To investigate the clinical effect of arthroscopic surgery in the treatment of anterior impingement syndrome of the ankle. Methods The study reviewed 28 patients with anterior impingement syndrome of ankle who were treated arthroseopically in our hospital from July 2011 to September 2015. There were 19 males and 9 females. The average age of patients was 31 years old （rang, 18 -49 years old）. There were 18 cases of soft tissue impingement and 10 cases of bony impingement. The effect of surgery was assessed by hind foot-ankle scoring of American Othopedie Foot and Ankle Society （AOFAS） and the Meislin criteria. Results All the 28 patients were followed up for 9 - 28 months, with an average of 18 months. The AOFAS scores were 38 - 65 points （mean, 49.2 ± 6.9 points） preoperatively and 72- 95 points （mean, 88.5 ± 5.5 points） at the latest follow-up, which were significantly improved （t = -26. 000, P = 0. 000）. Based on the Meislin criteria, the results were excellent in 20 eases, good in 5 cases, fair in 2 cases and poor in 1 case, the excellent and good rate being 89.3% （25/28）. There was no nerve injury and incision infection. Conclusions Arthroseopy is an effective measure for the treatment of anterior impingement syndrome of ankle. It is easy to perform, with reliable outcomes, fast recovery and high satisfaction rate, being worthy to be popularized in clinical practice.
