早产儿胃液培养检查在早期细菌感染诊断中的价值

摘　　要：目的通过分析早产儿胃液培养检查结果与早期细菌感染的关系，探讨早产儿胃液培养检查的临床意义。方法选择同期新生儿重症监护病房（NICU）中感染高危因素早产儿80例，均于开奶前行胃液培养检查，根据后续临床体征与相关临床检查结果将以上早产儿分为未感染组（n=37例）和感染组（n=43例），分析两组早产儿胃液培养检查结果。结果未感染组早产儿胃液培养阳性例数与感染组比较，差异有统计学意义（P〈0．01），早产儿胃液培养检查有较高的敏感性与特异性；导致早产儿早期细菌感染的高危因素在阳性早产儿的发生率均显著高于在阴性早产儿的发生率（P〈0．05）；早产儿早期细菌感染致病菌以革兰阴性杆菌为主，其次为革兰阳性球菌。结论早产儿应积极进行胃液培养检查，这对早产儿早期细菌感染诊断具有重要的指导意义，并可依据胃液培养检查结果，选择适合的抗生素及时进行有效的抗炎治疗。Objective To analyze the relationship between gastric juice culture and early bacterial infection in preterm infants and explore the clinical significance of gastric juice culture in preterm infants. Method 80 cases of preterm infants with high risk factors were collected in the neonatal intensive care unit （ NICU ） .They were divided into uninfection group （ n=37 cases ） and infection group （ n=43 cases ） according to the follow-up clinical signs and related clinical examination.The results of gastric culture examination were analyzed and the positive cases of gastric culture examination were analyzed. Results The positive rate of gastric culture examination in the infection group was significantly higher than that in the uninfection group （P〈0.01） .The gastric culture examination of preterm infants had a high sensitivity and specificity.The incidence of early stage bacterial infection in the infection group was significantly higher than that in the uninfection group （ P〈0.05 ） .The pathogens of early bacterial infection were predominantly Gram-negative bacilli and followed by Gram-positive cocci. Conclusion Premature infants should be actively carried out gastric culture examination, which early diagnosis of early bacterial infection has an important clinical significance, it can be based on gastric culture examination test results, then select the appropriate antibiotics in time for effective anti-inflammatory treatment.
