经皮穴位电刺激对妊娠相关性血小板减少症剖宫产患者围术期凝血功能的影响

摘　　要：目的:探讨经皮穴位电刺激(TEAS)辅助全麻对妊娠相关性血小板减少症剖宫产产妇围术期凝血功能的影响。方法:选取拟在全身麻醉下行剖宫产的患者80例,随机分为对照组和TEAS组,各40例,两组均采取静脉全身麻醉,诱导和维持方法相同。TEAS组在全麻诱导前30min选取双侧内关、合谷行穴位电刺激至手术结束,两组分别于麻醉诱导前(TEAS组在电刺激之前,T_0)、胎儿取出后20min(T_1)采集静脉血,进行血栓弹力图(TEG)检测。结果:TEAS组丙泊酚和瑞芬太尼用量比对照组明显降低(P<0.05);血栓弹力图各监测值组间比较:T_0时刻两组间各参考值比较差异均无统计学意义(P>0.05);K值T_1时刻TEAS组比对照组高(P<0.05);MA值T_1时刻TEAS组较对照组低(P<0.05)。组内比较:K值TEAS组T_1时刻比T_0时刻升高(P<0.05);MA值对照组T_1时刻比T_0时刻升高(P<0.05),TEAS组T_1时刻比T_0时刻降低(P<0.05)。结论:经皮穴位电刺激对妊娠相关性血小板减少症剖宫产产妇的高凝状态有一定的缓解作用。Ob je c t iv e ：To s tu d y Tran s cu tan e o u s elec trica lacup o in t s t imu la t io n （ TEAS ） on preg n an cy a s so c ia t-ed thrombocytopenia effect of blood coagulation function durning cesarean delivery perioperative. Methods： S e-lection methods in 80 patients with general anesthesia downlink of cesarean delivery, were randomly divided into con-trol group and TEAS group？ all the 40 cases？ two groups were taking intravenous general anesthesia？ induction and maintenance, in the same way. Observation group of 30 min before induction of general anesthesia selection closed in on both sides, or to electrical stimulation GuHang point to end the operation, respectively before anesthesia induc-tion （TEAS） before electrical stimulation （T0）,fetal after 20 min （T]_ ） venous blood was collected and tested for blood clots elastic graph （TEG）. Results：Two groups of general situation and transfusion blood loss without statisti-cal significance; TEAS g ro u p of p ropofol and fen tan y l d osage s ignif icantly lower th a n th e co n tro l gro u p , has s ta t is -tics meaning （P〈0 . 0 5 ）; th romb u s elastic figu re th e i r m o ni tor in g values b etwe en g ro u p s： th e re was no s ta t is t ic a l significance between the two groups at TO moment ;K value time Tx , high TEAS group than the control group, MA value time T]_,TEAS group was low,have statistical significance （P 〈0. 0 5 ） . G ro up com p a r iso n： K value T EA S group higher than T0 Tx time mome n t , was s ta t is t ic a l ly signif icant （P 〈0. 0 5 ） ; MA value o b se rv a t io n g ro u p th an the T; l t ime T0 momen t edged higher ,TEAS g ro u p tha n the T ; l t im e T0 t im e was re d u c e d ,had s ta t is t ic a l significance （P 〈0. 05）. Conclusion：Transcutaneous acupoint electrical st imulat ion can relieve pregnancy associated thrombocy-topenia associated maternal high coagulation state.
